SE Criterion # 7 - Transfer of parental rights at age of majority and student participation and consent at the age of majority

Basis for Findings:

A review of student records and interviews with staff members demonstrated that the district provides notice and options for the transfer of parental rights to the student and the parent at the Team meeting one year prior to the student reaching 18 years of age. When a student reaches 18 years of age the district implements procedures to obtain consent from the student, student and parent or delegated adult in accordance with the student's selection regarding decision-making.

SE Criterion # 8 - IEP Team composition and attendance

Basis for Findings:

A review of student records and interviews with staff members showed that the district maintains procedures consistent with the requirements for Team attendance at each IEP meeting. Team chairs have received training and are implementing the excusal process that includes parental agreement and a written report from the absent required Team member.

SE Criterion # 18A - IEP development and content

Basis for Findings:

Student record review and interviews with staff members demonstrated that the IEP Team considers and addresses the skills and proficiencies needed to avoid and respond to bullying, harassment, or teasing for students identified with a disability that affects social skills development or when the student's disability makes him or her vulnerable to bullying, harassment or teasing, as well as for students identified with a disability on the autism spectrum.

SE Criterion # 18B - Determination of placement; provision of IEP to parent

Basis for Findings:

A review of student records and interviews with staff members demonstrated that the district consistently provides a comprehensive summary form that outlines the decisions and agreements made at the meeting and completed IEP Service Delivery Grid to the parent at the conclusion of each IEP Team Meeting. The review of records also showed that well within two calendar weeks of the IEP Meeting date, the parents receive the Parental Notice form (N1) and two copies of the proposed IEP.

Basis for Findings:

Interviews with staff members and a review of student records demonstrated that the district has procedures in place to address when a parent revokes consent to special education services in writing. No parents have revoked consent for services at this time.

SE Criterion # 26 - Parent participation in meetings

The district provided the student roster documentation required by the Department.

SE Criterion # 29 - Communications are in English and primary language of home

Rating:

Basis for Findings:

A review of student records and interviews with staff members demonstrated that the district has a formalized system for ensuring that communications are in the primary language of the home for low incidence languages. Currently translations are provided in Chinese and Arabic in addition to Spanish and Portuguese. The special education administrator and principals also ensure that oral translations are documented in the student record.

Interviews with staff members and an onsite observation demonstrated that students in the

Supported Inclusion Program at Forest Elementary School eat lunch in the cafeteria and are fully included in with their general education peers.

SE Criterion # 55 - Special education facilities and classrooms

Basis for Findings:

On site observations were conducted in five district schools to verify corrections for the facilities and classrooms used for special education and related services. At the Mulready

Elementary School, physical therapy is now provided in a dedicated space, Room 15A. At the Hubert Kindergarten School, special education and related services are all provided in assigned appropriate classroom spaces and staff no longer uses the foyer. The Forest

Elementary School has removed the shared space so that special education and English language learner classes now have their own individual spaces. At the JFK Middle School related services have been allocated appropriate space. At Hudson High School, the STRIVE program for high school students has been moved out of the preschool wing into a classroom that has access to an area for independent living skill building and maximizes the inclusion of students into the life of the school.
