Plus/Delta Mentoring Form
A Plus/Delta is an evaluation tool. This Plus/Delta is for the mentee and the mentor to
complete separately as an evaluation of the mentoring experience. The Plus column is used to
record what was done well. The Delta column is used to record what could be changed and
most importantly, how the changes could be made. Please return the completed form to the
administrative coordinator in your school.
