Thorne Prize for Social Innovation in Health or Education
Stage 2: Written Application
The Thorne Prize for Social Innovation in Health or Education is a $25,000 cash
prize awarded to the best student-led venture focused on social innovation in
health or education. The prize will be awarded in the spring of each year to
support a student or group of students beginning or continuing to develop a
social enterprise or innovation focused on health or education.
Title of the Idea:
This application is to be completed and submitted as part of Stage 2. There are two easy steps
to complete your Stage 2 Application submission: (1) complete the Entry Agreement Form and
(2) complete this Stage 2 Written Application. If you already submitted your Stage 1 Letter of
Intent and the Entry Agreement Form, then you only have to complete the Stage 2 Written
Application.
Please complete all relevant information below. Submit by e-mail to the program
administrators. To submit, please save this file as [Title of the Idea]_Thorne Prize_ Round
2_[Date].doc and the Entry Agreement as [Title of the Idea]_Thorne Prize_ Entry
Agreement_[Date].doc and then e-mail it to Martin Klein (ihy@yale.edu) by 11:59PM EST on
March 28, 2016.
