Complete the Rockville High School “Transcript Request Checklist” 2. Obtain parent and student signature 3. Turn the form in to Ms. Mendez, Counseling Secretary How to Obtain a teacher recommendation We encourage students to approach teachers in person when asking for a recommendation.
