1. The RFP for Construction Inspection Services (Project No. 14-07) is hereby amended as set forth below:
- Due date is extended as below in section 4.
- Reference to Attachment 8 has been removed in section 13.
- Exhibit 3 Price Proposal is added to the RFP
I. REQUEST FOR PROPOSALS - Summary
4. Proposal Due Date/Time:
HCC will accept sealed proposals in original form to provide the required Construction Inspection Services until
3:00 PM (local time) on January 28, 2014. Proposals will be received in the Procurement Operations Department,
3100 Main Street (11th Floor, Room 11A06), and Houston, Texas
77002.
13. Conflict of Interest:
If a firm, proposer, contractor, or other person responding to this solicitation knows of any material personal interest,
direct or indirect, that any member, official, or employee of HCC would have in any contract resulting from this
solicitation, the firm must disclose this information to HCC. Persons submitting a proposal or response to this solicitation
must comply with all applicable laws, ordinances, and regulations of the State of Texas Government Code, including,
without limitation, Chapter 171 and 176 of the Local Government Code. The person /proposer submitting a response to
this solicitation must complete (as applicable), sign and submit Attachment No. 7 , Conflict of Interest
Questionnaire Form with the proposal package. HCC expects the selected contractor to comply with Chapter 176 of
the Local Government Code and that failure to comply will be grounds for termination of the contract.

Submit one sealed envelope to contain one (1) complete copy of your firm’s hourly rate schedule and the
Exhibit 3 Price Proposal with the following clearly marked on the outside: “Price Proposal – (firm name).
Project Name, RFP Number”.
Provide Price Proposals as follows:
1. Provide hourly rates schedule for all positions that will provide services under consideration as they would be
billed to HCC. With these hourly rates please indicate the job title, a short corresponding job description and the
service they would provide. The prices are to be inclusive of all costs including transportation, communication,
report preparation/duplication, postage & handling.
