FORMS TO BE SUBMITTED TO THE BALTIMORE COUNTY 4-H OFFICE
*Please Note: 4-H Clubs and programs may lose their charter if club forms are not returned. We
appreciate your cooperation!
ITEMS DUE ON OR BEFORE DECEMBER 31 OF THE CURRENT YEAR


Member Enrollment & Re-Enrollment:
 New members will need to enroll on Maryland 4HOnline at
http://maryland.4honline.com. Club Leaders will have directions on how to enroll on
the 4honline system or you may get directions from the Baltimore County 4-H Office
at 410-887-8090.
Maryland 4-H Behavioral Expectations

4-H Behavioral Expectations are now online. Youth and UME Volunteers
need to complete them when enrolling or re-enrolling.
UME 4-H Volunteer Re-Enrollment:
 To be completed online by approved UME Volunteers ONLY. Each
Volunteer should have a family profile that he/she created during the initial
enrollment. A paper form will be available by special request only.
 To qualify as an UME Volunteer, a person must have completed a UME
Volunteer Application and executed a UME Volunteer Appointment
Agreement form. A person remains a volunteer only for that period specified
in the Volunteer Appointment Agreement. Persons not meeting the above
requirements are not UME Volunteers and are not covered under the
Maryland Tort Claims Act.
The following must be completed to become a UME volunteer:
1. Application
4. References Returned
2. Position Description
5. Contract Signed
3. Completed Training
6. Interviewed by 4-H Office



If an adult has not taken UME training they are not considered a volunteer and
may only work with their own child. We recommend all parents take UME
training in order to help with other children in the club.
Club Information Form (orange):
 Information should reflect the program beginning January 1 to December 31
of the New Year. Changes and additions must be called into the 4-H office.
Club By-Law Sheet (light green): (ADDITIONAL LANGUAGE INCLUDED THIS
YEAR)


This is located on the back of the “Club Information Sheet: Please attach a
copy of your by-laws!! All clubs must have by-laws and these must be in our
files.
AA-AF 13 Form:
 This form stating you have open membership must be signed and returned
with your other club records. Only return one completed copy and keep one
copy for your club file.

4-H Club/Group Meeting Calendar:
 This plan will help you organize your programs and set your clubs goals for
the year. We realize that some of these plans will change. You should send
one copy of this form to the 4-H office and keep one copy for your club
records.
Maryland 4-H Charter Application or Renewal:
 All chartered clubs submit renewal. All new clubs submit
application

Maryland 4-H Foundation Group Affiliation Agreement for Clubs: (one time)
ITEMS DUE JULY 1 OF THE CURRENT YEAR

Public Relation Efforts by Clubs: Volunteer leaders and 4-H members are asked
to assist in making contacts and help promote the 4-H program. This report
(AA-4-HA) is for your promotional efforts. Make sure the following statement is
on any club flyers, posters etc.: “The University of Maryland Extension programs
are open to any person and will not discriminate against anyone because of race, age,
sex, color, sexual orientation, physical or mental disability, religion, ancestry,
national origin, marital status, genetic information, political affiliation, and gender
identity or expression..” “If you have a disability that requires special assistance for
your participation in this program please contact _________________________.”

Property Inventory List (Required by ALL clubs)

9990-N e-Postcard: (REQUIRED by ALL CLUBS) Please print the screen after filing and
return to the 4-H office

A 990-N e-postcard must be sent to the IRS for the tax season July 2014-June
2015. To find the 990-N e-postcard go to: http://epostcard.form990.org/
ITEMS DUE MONTHLY

Meeting Post Cards:
 These may be done by any club officer or volunteer and e-mailed to
Wendydil@umd.edu. Highlight your clubs programs, community service
projects, club demonstrations, or other interesting facts or accomplishments
with 4-H’ers or your club.
FIELD TRIP AND OVERNIGHT FORMS

Consent forms and Health forms must be completed by 4-H and adult participants at
least 2 weeks prior to any 4-H field trip or overnight event. Each event should be
included on the annual club calendar. Please work with the 4-H office to tailor
consent forms for each event.
NOTICE: If all forms are not turned into the office on time we cannot renew your club’s affiliation for the
New Year.
