AFFIDAVIT
I, _____________________________________________ hereby understand that:
a) It is MY SOLE RESPONSIBILITY to have all my courses approved prior to my
departure for study abroad.
b) If I make any changes to the courses while abroad I will be responsible for
contacting via email an OISP director/advisor and a college faculty/advisor for
approval of the changed courses immediately on registration at the
university.
c) I understand that all my study abroad courses will be posted to my transcript but
only those courses that have been pre-approved will be considered to satisfy my
degree requirements. I will need an approval for all courses on my transcript.
d) I understand that it is my responsibility to apply for scholarships and financial aid
for study abroad programs and that I will utilize the award received towards the
applied study abroad program.
e) I understand that my grades and credits will count towards my GPA as
letter grades.
f) I understand that it is my responsibility to complete my RE-ENTRY requirements
with OISP within 10 days of completion of program and I am responsible for my
transcript submission.
In the event the above point (e) does not occur within the following
semester then I will receive “F” grade for my study abroad courses.
g) I understand that I will not be able to graduate unless all outstanding tuition, fees
and program costs are paid to the study abroad provider/university/school.
h) It is my responsibility to check that my account does not have any holds in order
to be enrolled at UH by OISP and that I will need to submit to OISP my approved
General Petition for the overload of courses.
i) I have read and understood all the required forms and materials for study abroad.
