REQUIREMENTS:
The following 4 criteria are REQUIRED in order for the Office
of Financial Aid to process your loan within four weeks.
When your application is reviewed and the 4 criteria have
not been completed, your application will not be processed.
The Office of Financial Aid does NOT return any incomplete
applications. Check with the Office of Financial Aid after 20
business days to follow up on your application status.
Have a valid 2015-2016 FAFSA Application
(www.FAFSA.ED.GOV)
Must be a matriculated student, registered for at
least 6 units, within your grade level, per semester
during the 2015-2016 academic year.
Must have applied for the ‘William D. Ford Federal
Direct Stafford Loan’ and have been approved for
the annual limit of $20,500 (unsubsidized) for the
2015-2016 academic year.
Complete a Master Promissory Note (MPN) **

-‘Graduate Plus’ option
* If you do not complete the MPN, your loan will
not be disbursed
Must complete and submit the ‘Consent to Obtain
Credit Report’ form with this application

Approval of a plus loan is determined by credit worthiness.
The applicant cannot be delinquent on the repayment of any
debt or the subject of a default determination, bankruptcy
discharge, foreclosure, repossession, tax lien, wage
garnishment or write-off of a Title IV debt during the last five
years. Visit www.Studentloans.gov for more information.
PROCESSING TIME:
Processing time for a loan increase is four weeks. The Office
of Financial Aid will post a “Do Not Cancel” (BCF) service
indicator on your bill if you have an outstanding tuition
charge for the semester. You can view the BCF service
indicator on you CUNYfirst account under Student Services
Center. Once your increase request is processed you can
view the award under Student Services Center, click on
“financial aid” tab.
INCREASING YOUR GRADUATE PLUS LOAN:
You must submit a NEW Graduate Plus loan application if
you are requesting additional funding. You must also
complete another Graduate Plus Master Promissory Note.
Please allow four weeks for your request to be processed.
CANCELLING/REDUCING YOUR GRADUATE PLUS LOAN:
The Graduate Plus loan may be reduced or cancelled by
submitting the 2015-2016 Reduction/Cancellation Form.
Processing time is 4 weeks. Loans that have already been
disbursed must be returned to the Bursar’s office or the loan
lender. If the loan is reduced or cancelled, the student may
be responsible for part or all of their tuition.
NSLDS WEBSITE: Students should use the NSLDS website to
keep track of their loan borrowing such as aggregate limits.
NSLDS provides you with your loan history for both

The Privacy Act of 1974 (5 U.S.C.552a) requires that the following notice be provided to you. The authority for collecting the information requested on this form is §451 et
seq. of the Higher Education Act of 1965, as amended. Your disclosure of this information is voluntary. However, if you do not provide this information, you cannot be
considered for a Direct PLUS Loan. The information on this form will be used to determine your eligibility for a Direct PLUS Loan. The information in your file may be
disclosed to third parties as authorized under routine uses in the Privacy Act notices called "Title IV Program Files" (originally published on April 12, 1994, Federal Register,
Vol. 59 p. 17351) and "National Student Loan Data System" (originally published on December 20, 1994, Federal Register, Vol. 59 p. 65532). Thus, this information may
be disclosed to federal and state agencies, private parties such as relatives, present and former employers and creditors, and contractors of the Department of Education
