Your Review Meetings need careful preparation, and this form is designed to help that process.
June Review
This is your main PDR review.
Part A: Taking Stock - to review achievements and development over the past year
Part B: Personal Effectiveness - to assess your performance against the Competency Framework and identify development needs
Part C: Planning Ahead - to agree plans for the next year
January Review
This is an opportunity to monitor actions agreed in the June review and update plans in light of any changes.
The stages of the review process
The conversation with your Reviewer is at the heart of the process; the form serves as a record of the two meetings. You may find the following
checklist helpful:
 your reviewer will contact you to arrange a meeting
 complete the relevant sections: Self assessments etc
 pass the form to your Reviewer two weeks before the meeting
 ensure that the outcomes of the discussion are recorded and that you and the Reviewer have signed the form
 a copy of the form should be passed to your Head of Service/College

This section provides you with an opportunity to say how effectively you carry out your job. You should focus on where you believe you are
performing well and where you would like more support or development (you do not need to consider all competency areas).
