Global Youth Summit on the Future of Medicine Terms, Conditions & Release TUITION Student and student’s parent or guardian (“parent”) understand and agree that the student’s application must be received by January 15, 2016. In the event that the student is admitted to the Summit, the student and parent will submit a check or authorize a credit card transaction for a non-refundable deposit of $280 or pay the $2980 tuition in full to secure the student’s enrollment. All credit card transactions will appear as “Brandeis University.” Tuition must be paid in full no later than May 15, 2016 or delegate’s enrollment will be canceled and any refund will be subject to the cancellation and refund policy detailed below. Student and parent acknowledge and agree that participation entitles the delegate to programming scheduled by the Global Youth Summit, including, but not limited to, room, board, ground transportation, activities and all educational materials and events. Brandeis University will comply with all applicable ADA requirements for delegates needing reasonable accommodation. BRANDEIS UNDERGRADUATE SCHOLARSHIP Student and parent understand that an added benefit of successful completion of the Summit is a $3000 scholarship awarded to the delegate should s/he eventually be admitted and enrolled as a freshman undergraduate student at Brandeis. Delegate and parent understand that participation in the Summit does not guarantee admission to Brandeis as a degree-seeking student. ADMISSION & WAITLIST Student and parent acknowledge that this is an invitation to apply for admission to the Global Youth Summit. There is a nonrefundable application fee of $35 to apply to the Summit. Brandeis University has full discretion on the admission of all delegates. All admission decisions will be made by February 2, 2016. The Global Youth Summit does maintain a waitlist for qualified delegates. DELEGATE HONOR CODE Student and parent understand the delegate will agree to and sign the Delegate Honor Code at the delegate’s first group meeting, and any violations of the Honor Code may result in dismissal from the Summit at the delegate and parent’s expense. PUBLICITY & RESEARCH Student and parent grant Brandeis University and the Global Youth Summit the authority to use still, audio, and video recordings taken of the delegate and quotes from evaluations in future publications and for educational research undertaken by the university. PHONE USE Student and parent approve the use of a cellular phone by the delegate and understand that delegates may only use their cell phones according to the rules established in the Delegate Honor Code. GROUND TRANSPORTATION Student and parent authorize Brandeis University, its agents and assigns to provide transportation to the delegate in connection with the delegate’s participation in the program MEDICAL AUTHORIZATION Separate and apart from the admission application, delegate and parent agree to complete a Medical History and Emergency Medical Authorization Form upon delegate’s acceptance into the program. COMPUTER USE Student and parent acknowledge that delegates will have limited access to the internet and understand that, due to the nature of the internet, responsibilities rest with the delegate. The network is a public marketplace where nothing is hidden and Brandeis University cannot control what delegates do, except through the Delegate Honor Code and trust. Violation of the Delegate Honor Code will result in termination of the right to access the internet and may result in dismissal at the delegate and parent’s expense. Delegate and parent hold Brandeis University harmless for any violations of internet protocol detected or otherwise incurred by the delegates. RESPONSIBILITIES Student and parent agree to indemnify, release and hold harmless Brandeis University and the Global Youth Summit and their directors, officers, employees, agents, representatives and affiliates from loss, liability, damage or expense arising out of the delegate’s participation. Brandeis University shall not be liable for loss, damage or expense arising from cancellation, natural disasters, governmental actions or any other occurrence beyond Brandeis University’s control. This agreement shall be interpreted under the laws of the Commonwealth of Massachusetts. CANCELLATION & REFUND Student and parent understand that cancellation and refund requests must be made in writing and sent via email to youthsummit@brandeis.edu. • Cancellation and refund requests must be received no later than May 15, 2016. The $280 deposit is non-refundable. • Cancellations made prior to May 15, 2016 will be refunded all tuition, less the $280 deposit. • Cancellations made after May 15, 2016 will forfeit all tuition paid. • Delegates who are dismissed from the Global Youth Summit for violations of the Delegate Honor Code or leave the Summit early for any reason are not eligible for refund. PROGRAM MODIFICATIONS Brandeis University and the Global Youth Summit reserve the right to postpone, modify, or cancel the Summit at any time. If the delegate elects to cancel from a postponed or modified Summit, the $280 deposit is forfeited. If Brandeis University cancels the Summit, all monies will be returned, but Brandeis will not be responsible for any additional costs incurred, other than those covered under “Tuition.” EQUAL OPPORTUNITY Brandeis University complies will all applicable federal and state laws and admits qualified delegates without any regard to race, color, sex, religion, nationality, disability or sexual orientation.
