TMCC DA student participation in dental practice/OPA options Attending: Julie Muhle, Linda McGillicuddy, Ted Plaggemeyer, Dr. Karen West, Dr. William Wager and Dr. Tony Guillen. Meeting focused on issues surrounding the initiation of a faculty dental practice at TMCC. At UNLV, in the Faculty practice Hygienists and Assistants are employees. The dentist gets 40 % of usual and customary and the school gets 60%. The dentist pays his/her own lab bill. The school has a blanket malpractice insurance policy that did not change regardless of docs, residents, or students doing the dentistry. We should check out this company. Even doing pedo and adding anesthesia did not make it go up. For the students doing dentistry, if it gets too complex the dentist takes over. Accreditation ratios must be maintained. Find out what they are. Dr. Guillen says that faculty practice won’t work here because not all are licensed in NV, some have special/limited licenses to do certain things, like specialties. He says that we need students and they will need room, board, and travel, but don’t get paid for services. We need to make sure the supervising dentist is calibrated with UNLV. The students need the biggest bang for their buck. They need the most, best experience in the shortest amount of time. Double booked. They need incentive and it needs to be easy. He suggests a ratio of no more than two students to one doc. Dr. Wager agreed to go to UNLV for calibration. Dr. West suggests that an association like AHEC might be able to help with the student’s needs. The student in Elko was most likely housed with the medical students. UNLV might also have some money that can help support a student. Students will most likely rotate for two-week periods. They would make sure that students who need the least supervision would be sent. The concern is that we may not be able to do certain procedures that require lab work because of the limited time the students are here. It could be put into curriculum as externship and could be elective at first, again a community outreach sort of thing, having them research the community, etc. There will be no fees between us and UNLV. Dr. Wager suggests contracting with Indian Health Services, the VA, or HAWC. We need to market it as trying to serve those falling between the cracks. We must get an affiliation agreement. Make Dr Wager faculty of joint appointment. We have scheduling problem in the clinic as it is. Three or four chairs in the Materials lab would be the simplest solution. Would we need a full-time assistant? We couldn’t rotate ours if they are supposed to be in class. We would need a compressor. We would have to have a special grading program that is tied into UNLV. Who would handle the insurance billing and administrative stuff? At this point we need to speak with the Hygiene Director regarding clinic time. Dr. West thinks it would be great to get the dental students involved in Hygiene student evaluations as well. Someone suggested looking into surplus equipment and at offices that are remodeling like Dr. Logan Smith. Dr. Guillen says we need to be careful who the faculty members are and the need for high ethics, work quality, and good chairside manner. Julie asked Dr. Guillen what he thought of the OPA and expanded functions. He replies that it is coming. The problem with requiring CDA certification is that rural docs have enough trouble getting assistants and the expense of licensure is high. He used docs and hygienists’ licensure as an example. We don’t want to be like California. Dr. Guillen says to check out CSN as some restorative is done there. Dr. West suggests that some interests in the community might be willing to donate room, board, car rental. Some other contacts were suggested: Job Corp, Lori Benvin, NV Dental Association, Larry Champaign, the Salas, Pascucci, and Muff. Will involve DH now since we have more info to work with and help with decision-making.
