Title: Patient and Community Collaborative for Cardiometabolic Health (PACT for Health)
Goal: In partnership with the International Society for Hypertension in Blacks (ISHIB), this project
proposes to develop strategic partnerships in health disparate communities (Camden, NJ; Baltimore, MD,
Orangeburg, SC; El Paso, TX) with federally qualified health centers (FQHCs), patients, churches,
businesses, and other local groups to identify and intervene upon key modifiable medical and social
determinants to improve cardiometabolic health.

Goal: The South Carolina Rural Health Research Center focuses on health and health care disparities

compiling and interpreting key social determinants of health among rural populations: demographic

Goal: The overall goal of this research proposal is to explore the relationship between public health equity
and civil rights advocacy focusing on the legacy of Modjeska Monteith Simkins. The specific objectives of
the project are to: 1) conduct key informant in-depth interviews; 2) examine historical and contemporary
primary and secondary sources; and 3) prepare and submit a manuscript to the American Journal of Public
