Signature Release and Authorization
I,
, an employee of Western Kentucky University, hereby agree and
authorize the staff of the Kentucky Teacher Internship Program office to
execute University Travel vouchers prepared for my reimbursable travel
expenses on my behalf.
I am not able to sign these original forms without significant delay in
processing. By allowing the above listed office to sign my travel voucher
and by accepting, processing and/or endorsing the reimbursement check, I
agree and understand that such actions will serve as my personal
verification and confirmation that all information on said vouchers is correct
and accurate.
I agree to the terms of this release and authorization.
