Subject to the provisions of the above-numbered Uniform Grain and Rice Storage Agreement (Agreement), this SCHEDULE OF RATES will become effective as of April 1 and supersedes all previous
SCHEDULE OF RATES. This SCHEDULE OF RATES continues in effect through March 31 of the following year, and continues in effect thereafter until superseded by a new SCHEDULE OF RATES accepted
by CCC and which will become effective on the next annual renewal date of the Agreement. For any subsequent rate to be considered by CCC, it must be submitted for approval at least 30 days prior to the
annual renewal date of the Agreement. Read the information instructions and fill in all blocks.
For commodities for which services are offered to CCC, indicate in the blanks the rate for such services. If a service for a commodity is not offered to CCC, put an N/A in the blank. The warehouse operator may
offer rates for services for a commodity to CCC at a later date if a rate is not offered at a contract renewal. Failure to place an entry in any block will indicate that the service is offered at no charge. Rejection,
negotiation, or acceptance of this schedule by CCC will be based on the total offer and not each element.

Footnotes 1 through 5 apply to grain accepted for storage and handling under this Schedule of Rates.
1/ Charges must be computed on the basis of net quantity of grain received in the warehouse. Net weight for soybeans is gross bushels minus foreign material in excess of 1 percent.
2/ On farm-stored deliveries, rates of "handling only" on an identity preserved basis will be negotiated or established by CCC.
3/ If identity preserved storage is necessary, storage charges will be negotiated on a case- by-case basis.
4/ When the uninsured storage rate for CCC-owned grain is the same or more than the insured rates, the warehouse operator must carry full market value insurance as described in Part III section H of the
Uniform Grain and Rice Storage Agreement, and the insured rate will apply.
5/ Include customary or mandatory wharfage charges. Bulk carrier loadout rate applies to ocean-going barges.

The authority for collecting the following information is Pub. L. 107-171. This authority allows for the collection of information without prior OMB approval
mandated by the Paperwork Reduction Act of 1995. The time required to complete this information collection is estimated to average 1 hour per response,
including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the
collection of information.
The following statement is made in accordance with the Privacy Act of 1974 (5 USC 552a). The authority for requesting this information is the Commodity
Credit Corporation (CCC) Charter Act (15 USC 714) and the regulations promulgated thereunder (7 CFR Part 1421). The information will be used to complete
the terms of an agreement between the warehouse operator and CCC. Furnishing the information is voluntary, however, without it, eligibility to enter into an
agreement with CCC cannot be determined. This information may be provided to the IRS, the Department of Justice, other governmental agencies, other State
or Federal law enforcement agencies, and in response to a court magistrate or administrative tribunal. The provisions of criminal and civil fraud statutes,
including 18 USC 286, 287, 371, 641, 651, 1001, 15 USC 714m, and 31 USC 3729, may be applicable to the information provided. RETURN THIS
