Creative Arts Scholarship 2016-2017
Visual Arts Application
Application Deadline: March 2, 2016
There are five renewable $3,000 scholarships available to incoming visual arts majors who are new to PUC.
I. Application Procedures
Please complete this application and mail it with the following items:
1. One letter of recommendation. The letter should be written by a teacher, pastor, counselor, or a person who
can assess your character as well as your academic performance. The letter may not be written by anyone
related to you and does not have to refer to your artistic ability in particular.
2. Eight samples of your artwork. Please number the works and list each with the title, medium and year of
creation on the back of this form. Please send digital works (1200 x 1500 pixels wide) on compact disc (pdf or
jpg), or email attachments sent to art@puc.edu.
3. A written statement of purpose (see back page for what to include.)
4. All application materials must be postmarked on or by March 2, 2016. Mail all materials to:

5.Recipients will be notified by April 1, 2016 and must confirm acceptance of the scholarship by April 7, 2016.

V. Arts Scholarship Agreement
I acknowledge with my signature below the above information is accurate and complete to the best of my
knowledge. I understand if I receive a scholarship I will be expected to be enrolled as a major in the department of
visual arts. I also understand the scholarship is for the entire year and one third of the award will be applied
to my bill for each quarter I am enrolled as a full-time PUC student.
