This form is for students enrolled in the Computer Science Doctor of Philosophy program. In order to register for Ph.D.
Dissertation Research Preparation (CS 8999) hours, you must have a permanent advisor but have not passed the qualifier.
Complete this form in consultation with your dissertation advisor. Bring the signed form to your Graduate Student Affairs
office. You will then be given a permit to enroll in the course. This form only needs to be completed once. See your staff
program advisor for more information.
