Instructions for Administrative Banner Class/Object Modification Request Form (v3.0) Assistance with completing this form is available from your ITSM (Information Technology Systems Manager) or the Information Security Office. If you need help with this form, please ask. It is far easier to have Information Security review and identify any concerns before all the approvals. Any changes must be initialed by the Administrator. Incomplete or inaccurate paperwork will be returned. Class Name: Class names that can be modified begin with EPCC_ or C_ and do not end in Obsolete. If you do not know the appropriate class name or if this is a new class, contact your ITSM or the Information Security Office. Information Security will modify the Class name as needed to conform to current processes. Action: Select from drop-down. New Classes require testing to ensure all needed forms will function without errors. Class Owner Information: Only Administrators can be class owners. Reason for Creation or update: For Creation, identify what department and departmental goals this class will support. Example: For the __ office to advise students and process registrations". For updating, identify what departmental goal/duties does this change support. "To do daily work" is too vague. "New fiscal year tables" or "To verify course completions" is acceptable. Production/Test Database: Production is the live database, also known as EPCC. Most classes only need EPCC. Object Name: If your request involves more than 15 objects, contact the Information Security office. Object names are usually 7-8 letters long and can be Banner forms, reports or scripts. Some Banner forms require other Banner forms to work properly. Table names are similar but require a different type of access. This can be confusing so when in doubt, please ask your ITSM for assistance before submitting the request. If the object name is inaccurate or missing, your request cannot be processed. Object options: Query means that the user can view but not update the information. Modify allows the user to change some or all of the information, depending on the object. When in doubt, use Query. If the form requires Modify (most reports do), the Information Security Office will make the change.. Signatures: Signatures from the Class Owner (Administrator) and VP are required. Be sure their name is printed since not all signatures are legible. ITSM signatures are highly recommended. Sending unsigned forms delays the process since they have to be returned. If an Office Supervisor or other individual wants to be notified when the action is complete, please indicate that in the Comments area at the bottom of the form. Data Owner: Data owners are those administrators whose departments are responsible for the accuracy and integrity of the data displayed by the object. Data Owner(s) are determined by the object(s) requested. If the Class Owner is the Data Owner, leave this blank. FERPA Officer: The FERPA officer is the Executive Director of Admissions and Registrar. The FERPA officer must approve all access to Student Data regardless of where the data resides. Other: Various laws (like HIPAA or PCI-DSS) or College policies (like Employee Record access, etc) govern some information. Additional approvals might be required. Comments: If an Office Supervisor or other individual wants to be notified when the action is complete, please include the name/email address in this area. If the request is in advance of a patch or upgrade being applied to the system, be sure to include that information to avoid unnecessary delays.
