7134 – QUALIFICATION AND TRAINING
7134.01 – Authority
The Federal Property and Administrative Services Act of 1949 (40 U.S.C. 471, et seq.) requires
each executive agency to train, test, and qualify vehicle and equipment driver’s or operators in
compliance with orders and directives issued by the Office of Personnel Management in the
Federal Personnel Manual chapter 930, subchapter 1, Motor Vehicle Operators (FSH 6109.41 FPM 930, subch. 1). Supplements applicable to the Forest Service are found in Agriculture
Property Management Regulations chapter 104, part 38, subpart 5 (FSH 6409.31 - AGPMR
104-38.5).
7134.02 - Objective
To communicate the current Motor Vechicle Accident (MVA) policy for the Northeastern Area,
consistent with the Northern Research Station (NRS) to ensure the safe, productive, and energy
efficient operation of fleet equipment.
7134.03 – Policy
This policy states the procedure for reporting Motor Vehicle Accidents (MVAs) in a government
owned vehicle (GOV) and personal owned vehicle (POV) while on official duty. It also states
the procedure for obtaining a government driver’s license.
7134.04 – Responsibilities
It is the responsibility of the employee to ensure the Supervisor is contacted as soon as possible,
and the accident is logged into the Safety and Health Information Portal Systems (SHIPS) in a
timely manner. SHIPS is mandatory for use on all MVA’s while on official duty.
Employees and Supervisors will ensure the Property Management receives the original
hard copies of all accident forms associated with the MVA. This includes the SF-91
(Motor Vehicle Accident Report), SF-94 (Statement of Witness), AD-112 (Report of
Unserviceable, Lost, Stolen, Damaged, or Destroyed Property), and any applicable police
reports. SHIPS will initiate the SF-91 form. All other forms will need to be completed manually
and can be found on websites: http://www.gsa.gov/Portal/gsa/ep/formslibrary and
http://www.ocio.usda.gov/forms/ocio_forms.html.
It is the responsibility of the Supervisor to ensure their employees complete refresher training
every three years. Supervisors will check all employees’ OF-346s, United States Government
Motor Vehicle Operator's Identification Cards annually to ensure all training is current. If an
employee’s driving privileges are suspended or revoked by the issuing State, it is the employee’s

responsibility to inform their direct Supervisor immediately. The Supervisor will then contact
the Employees Relations Advisor for assistance.
7134.1 - Qualification
Obtaining a Government Driver’s License.
When operating government vehicles or equipment, the operator must carry Form OF-346,
United States Government Motor Vehicle Operator's Identification Card, or a Forest Service
issued identification card indicating the type of vehicle or equipment the holder is authorized to
drive or operate.
The following forms must be completed in order to obtain a government driver’s license:
1. FS-7100-184 -Application for Authorization to Operate Government Vehicles and
Equipment.
a. The form must be completed by the employee requesting a government driver's
license.
b. Section 1 will be completed by the employee. Block 10 (weight) is not required. The
employee will sign and date block 17 and give the original form to their Supervisor.
c. Section 2 will be completed by the employee's Supervisor. The Supervisor will sign
and date block 23 of the original form and submit it to the Issuing Official. The Issuing
Official is designated locally and should be at the level of Assistant Director,
Field Representative or Project Leader.
d. Section 3 is not required.
e. Section 4 will be completed by the Issuing Official. After completion, the Issuing
Official will return the original form to the employee's Supervisor to keep for filing.
2. OF-346 - U.S. Government Motor Vehicle Operator's Identification Card.
a. The employee will complete the OF-346 (NSN: 7540-00-634-3999) and submit it to
their Supervisor for approval, along with the FS-7100-184.
b. The social security number and weight will be left blank on the OF-346. The
expiration date will be indefinite for permanent employees and an appropriate date will
be filled in for temporary/term employees. Card numbers will be sequential starting with
the year (ex: 2008-01, 2008-02, etc).
c. The Supervisor will ensure the OF-346 is correct, to include all restrictions and types
of vehicles/equipment listed on the back of the form.
d. The Supervisor will sign the back of the OF-346 as the Qualifying Official. By signing
as the Qualifying Official, the Supervisor is certifying that the employee has a valid state
issued driver’s license and the employee has completed all required training needed to
operate the equipment. If an employee is qualified to operate specialized vehicles (ATV,
snowmobile, etc), the equipment type will be listed in this section. For many specialized

vehicles, specific training by a certified training official is required. The FSH 6709.11,
Health and Safety Code Handbook contains a list of specialized vehicles that require
certified training.
e. The Supervisor will give the OF-346 to the Issuing Official. They will sign the front
of the form and return it to the Supervisor.
f. The Supervisor will give the original OF-346 to the employee.
g. The Supervisor will file the original FS-7100-184 and a copy of the OF-346 in the
employee's personnel file.
Motor Vehicle Accident (MVA) Reporting.
All operators of Government-owned or -leased motor vehicles will be tested, and certified
through State licensing programs. Special equipment drivers/operators shall be qualified, tested,
and certified though Forest Service programs. When operating government vehicles or
equipment the Form OF-346, United States Government Motor Vehicle Operator's Identification
Card, or a Forest Service issued identification card indicating the type of vehicle or equipment
the holder is authorized to drive or operate must be in the employee's possession. The
identification card may be issued with an indefinite expiration date subject to review as required
by the Federal Personnel Manual, chapter 930.
The following steps will be taken after an MVA occurs and it is entered into SHIPS:
1. The Supervisor will conduct an investigation of the MVA, with the help of the
Property Management Officer, if needed.
2. If the employee is found at fault, the employee will be required to complete a State Approved
Hands-On Driver training class within 30 days of being found at fault. The cost of this training
will be borne by the Area Unit that the employee is assigned. The employee will not be
authorized to operate a government vehicle until the above training has been completed.
3. The Supervisor will contact the Employee Relations Advisor for assistance with disciplinary
action, if warranted. Pending the outcome of the investigation, disciplinary action can range
from a letter of warning, letter of reprimand, suspension of driving privileges, or termination.
4. The Supervisor will inform their Assistant Director/Field Representative/Project Leader of the
circumstances of the MVA and the proposed actions.
