Provides ability to capture New Hire or Life Event Benefit
Enrollment for Employees in self service;
Allows the Benefits Administrator ability to review, track,
and apply records in self service;
Enhanced Dependent and Beneficiary data that can be
maintained directly by the employee;
New Beneficiary Allocation information added to support
Employee Insurance, Retirement programs, and other
benefits;
Incorporation of Life Insurance Premium calculations for
the employee to view during enrollment in self service.

employee (with multiple address selection through GTVSDAX);
Document Management link for the Administrator to attach Life
Event authorized documents;

Provides change reason for Life Events to reside on PDADEDN
 Establishes the time period for employee filing Life Event
 Allows for use across any deduction for maintenance purposes to
provide history of an event

Benefit Deduction Termination Reason Code Form
(PTVDREA)
Provides for a reason a dependent is being terminated from
coverage
 May only be accessed when Coverage End Date is supplied



Supplies the list of document an employee is required to supply to
the Benefits office for completion of a Life Event change

Provide the documents an employee must file with the specific Life
Event
 Controls the Benefit Groups to be presented during Life Event
enrollment (Health, Life, Retirement, etc.)
 Controls which benefits will be presented in sequence
 Provides controls whether Coverage or Allocations can be viewed or
updated by the employee

