Instructions: Please complete the following form for the person you wish to nominate. Self-nominations are acceptable. Submit the nomination via
email to Sheila Pratt, Division of Research and Sponsored Programs (RASP) at spratt@kent.edu.
Please indicate who you are nominating, give a one-line description or title to the nominee’s scholarly accomplishment(s), and in 500 words or less
describe the research and/or creative activity you relied on to make the nomination. You may cite specific accomplishments such as research
articles, books, performances, exhibits or the nomination may be for a career or cumulative body of work. The scholarly activity, however, must
have been accomplished substantially while the person was a faculty member at Kent State University.
RASP will contact each nominee and request their acceptance of the nomination, CV, and additional materials as necessary to support the
nomination.
