received; this award is for advising excellence, not popularity. Please be honest and accurate. Give specific examples where
possible. All nominations will be held in the strictest confidence. To ensure fairness, evaluators will not see the names of the
nominees. To help in this process, please list the nominee’s name only at the top of this form. Within the body of the
nomination, be non-specific (i.e., he, she, Prof. X, Mr. X, Ms. X). Thank you!
Please answer ALL of the following questions and give concrete examples to support your answers.
1. What are the characteristics of this person which make you consider him/her to be an
outstanding academic advisor? How does this person personify ‘excellence’ in advising?
2. Does the nominee go ‘above and beyond’ the call of duty in helping you, the student, grow and
develop, assess your interests and abilities, and receive accurate information? Explain.
3. Does the nominee show caring and personal concern for your (the student’s) development at
Kent State University in relation to academic and career goals? Explain.
4. Does the advisor have a helpful and positive attitude about advising students? Give an example.
