Assessments of acutely unwell patients in community settings, including history taking, mental state examinations and risk assessment. Developing crisis management plans. Ongoing monitoring of acutely unwell patients. Prescribing experience under consultant supervision. Liaison with GPs, community mental health teams and in patient services. Working within a multidisciplinary team. Experience of a broad variety of mental health conditions. Chase Farm Hospital Dr Helen Moorey Attendance at daily team planning meetings. Home visits for assessment and ongoing treatment with the multidisciplinary team. Prescribing under consultant supervision. Teaching of medical students. Daily work to be allocated at morning team meeting. The majority of work will be in patients’ own homes. The employer for this post is Royal Free London NHS Foundation Trust - Barnet Hospital.
