Enclosed you will find the information needed to register for the
2015 ADULT FALL VOLLEYBALL LEAGUE.
1.
To register, please complete a roster and email it to me.
If you do not have an electronic roster, email me and I
will send you one to complete and return.
2.
Please complete all information in yellow on the roster; if
available, otherwise at least the name and email
information of players.
3.
The Registration Fee is $245.
4.
All Team Registration Fees must be paid either online (Visa
or MasterCard) or in the form of a company/bank/personal
check or money order.

5.
Any entries submitted without the above information listed
under #1, #2, #3, #4 will not be accepted and will be
returned.
6.
There will be NO REFUNDS after you register.
(EXCEPTION: If the League in which you apply is full or
not enough teams sign up to have the league, then a refund
will be issued.
7.
The first seven (7) teams that register in each division
will be placed in the Adult Fall Volleyball League.

All teams who have participated in 2015 MCPRD volleyball programs
will be allowed to register during the ‘early bird’ registration
period. Teams will be allowed to register into the six (6) slots
of the league in which they participated in during the year. The
dates for ‘early bird’ registration are now through August 24,
2015; after which, remaining spots will be filled on a first come,
first serve basis by new teams.
When your team is confirmed into the league, a confirmation email
will be sent to you, so make sure to email me back to confirm your
spot. I will then email you a blank roster to fill in all players.
All players must have reached their sixteenth (16th)
birthday before the first match of the season.
All persons participating on your team must be rated the
same level or lower than your team level. Any higher level
player who participates on a lower level team will be
subject to removal from the league at any time during the
season that this person is discovered. The final decision
will be made by the Athletic Coordinator.

MECKLENBURG COUNTY PARK & RECREATION DEPT (MCPRD), AGENTS,
EMPLOYEES, OFFICERS, REFEREES, AND FACILITIES OF THE MCPRD FROM
ALL CLAIMS, DEMANDS, ACTIONS, AND JUDGMENTS WHICH I MAY HAVE, OR
CLAIM TO HAVE, AGAINST THE MCPRD FOR ALL PERSONAL INJURIES, AND
ALL INJURIES TO PROPERTY, BOTH REAL AND PERSONAL, CAUSED BY, OR
ARISING OUT OF, PARTICIPATION IN GAMES,OR OTHERLEAGUE/TOURNAMENT
SPONSORED FUNCTIONS BY THE MCPRD, IT’S AGENTS, EMPLOYEES, AND
OFFICERS.
I HAVE NO PHYSICAL CONDITION THAT WOULD PREVENT ME FROM
PARTICIPATING IN THE LEAGUE/TOURNAMENT SPONSORED BY THE MCPRD.
I AM IN GOOD HEALTH AND PHYSICAL CONDITION. I FULLY UNDERSTAND
THE DANGERS INVOLVED IN THIS TYPE EXERCISE, FUNCTION,
COMPETITION AND PRACTICE.
THE MCPRD RECOMMENDS ALL PARTICIPANTS HAVE A HEALTH
PHYSICAL OR CONSULT YOUR DOCTOR IF YOU ARE UNSURE OF YOUR
PARTICIPATION IN THIS TYPE ACTIVITY IT IS THE RESPONSIBILITY OF
THE INDIVIDUAL PLAYER OR PARTICIPANT TO MAINTAIN HIS/HER OWN
HEALTH AND ACCIDENT INSURANCE. THE MCPRD ACCEPTS NO
RESPONSIBILITY IN THIS MATTER. THE MCPRD IS NOT RESPONSIBLE FOR
ITEMS LOST OR STOLEN; OR PROPERTY DAMAGE
