Annual Chemical Inventory Submission
Weill Cornell Medical College (WCMC) is required to maintain an inventory of all hazardous chemicals and report
annually on the types, quantities and storage/use locations at WCMC. Reporting must be made in compliance
with the NYC Department of Environmental Protection, NYC Fire Department, and US Environmental Protection
Agency Community-Right-to-Know requirements as well as US Department of Homeland Security Chemical
Facility Anti-Terrorism regulations. The inventories provide government agencies with information to plan for
potential chemical releases, and allow the agencies to be aware of hazards present during emergency response
activities. Supervisors of all locations storing and/or using hazardous chemicals must maintain a chemical
inventory on the web-based ChemTracker Chemical Inventory System.

To comply with the annual submission requirement, complete the following steps and submit the completed
confirmation form to EHS at chemtracker@med.cornell.edu by December 31st of each year.
STEP 1: CHEMICAL OWNER CONTACT INFORMATION
Chemical Owner Name (e.g., Principal Investigator, Director)
Department/Division
STEP 2: REVIEW CHEMTRACKER CHEMICAL INVENTORY
 Review the chemical inventory in ChemTracker for accuracy and update the inventory as needed.
 Select the appropriate chemical inventory status from the options listed below.
ChemTracker Chemical Inventory Status (select the most applicable)
ChemTracker chemical inventory review is complete and inventory is accurate.
ChemTracker chemical inventory is managed under another chemical owner’s chemical inventory.
Provide chemical owner’s name:
ChemTracker chemical inventory needs to be created. ChemTracker New User Request Form will
be submitted to EHS to create a new account and chemical inventory will be completed.
ChemTracker chemical inventory is not required since I do not store or use chemicals.

The ChemTracker chemical inventory status provided above is accurate to the best of my knowledge.
