Provider Quick Tips: Code Status StarForm within StarPanel Accessing the Electronic Code Status Form in StarPanel Click on Code Status banner in header of patients chart 1 Click on Code Status in header (will only display if Code Status form needs your signature (attending's) From Forms: • Open pt chart • Click Forms yellow tab • Type code in search field 2 3 Click on any blue ACTIONS for patient From menu: • Click Code Status form to start new form • Click Code Status Hx to display history of code status for patient 4 Definitions of Code Status Code Status DNR DNR/DNI FULL Cardiopulmonary Deterioration Before Arrest Provide all medically appropriate care not refused by the patient/surrogate/family Do not intubate but provide all other medically appropriate care not refused by the patient/surrogate/family Provide all medically appropriate care not refused by the patient/surrogate/family Pulseless Cardiopulmonary Arrest Do not attempt resuscitation Do not attempt resuscitation Provide all medically appropriate care not refused by the patient/surrogate/family ****Please note that if a patient is found in a pulseless cardiopulmonary arrest, DNR and DNR/DNI are the same, do not attempt resuscitation. The difference between DNR and DNR/DNI is highlighted only when a patient is experiencing cardiopulmonary deterioration before arrest (e.g. actively declining). For the actively declining patient, DNR/DNI confers the ability to not intubate a patient (e.g. lung cancer, end stage COPD) but provide all other care. Message alert displays upon signing into StarPanel a. Click OK - list of all Code Status forms needing confirmation by provider will display b. Click Review to open form c c. Click Mark as Confirmed a b Once marked as confirmed – indicator changes to reflect a confirmed status 2. Click yellow or red indicator from patient list in StarPanel a. Form Opens b. Click Mark as Confirmed 3. Click Code Status at top of StarPanel – list of all Code Status forms needing confirmation by provider will display a. Click Review to open form b. Click Mark as Confirmed Change the Approving Attending to Another Attending 1. Go to All Documents list in StarPanel a. Search for Code Status forms by typing Code in search field b. Click on form name to open c. Click Send to a different attending to approve d. Type name of attending/select from list & click Send e. Message displays Re-routed to (attending name) StarPanel Indicators for Code Orders Code Status displayed as “unconfirmed: (last code status) (date)”- (see screen shot below) • If Code Status established on prior admission and not reestablished on current admission • IF Code Status established and not signed by an attending within 48 hours • Charge Nurse will monitor indicators & and alert team if attending signature needed • Hover over indicator for details • Click on indicator to open most recent Code Status Form Code Status Downtime Procedures • • • If StarPanel Downtime : Obtain paper Code Status Form from medical receptionist or nurse Complete all sections of form including signature section If completed by Houseofficer/APN/PA - must designate attending & obtain signature After downtime • For the indicators and electronic notifications to work, this information must be entered into StarPanel once downtime is over.
