This document contains a listing of Northern California Resources gathered by Lisa Kjer, LCSW at the HDSA Northern
California Chapter and HDSA Center of Excellence at UC Davis Medical Center. The resources listed are for patients,
families and the HD community to assist in finding the community support needed to cope with Huntington’s Disease.
Majority of the resources in this document are not affiliated or funded by UC Davis Medical Center or HDSA Northern
California, therefore many of the resources are not specific to HD, yet may be useful to HD patients, families and
community members. It is our hope that these resources may assist you in some way.

Adult Day Health Care
Adult Day Health Care is licensed community based day care programs offering services such as medical services,
physical therapy, occupational therapy, speech therapy, personal care services, social services, nutritional counseling,
meals while at the center and transportation to and from center. ADHC services are either paid by Long Term Care

Participation in therapy and counseling can be very beneficial and assist with coping, problem solving, communication,
grief/loss, stress management and much more. There are many types of counselors/therapists (LCSW, MFT,
Psychologist, etc) and you will need to choose a therapist that you trust and are comfortable with. Additional
counselors/therapists may be available to you through your health insurance. Please contact your insurance provider’s
behavioral health services for a listing of counselors/therapists that accept your insurance. Counselors/therapists will

Hospice
Hospice care offers services and therapies to patients that are terminally ill and want to remain at home. Hospice is a
concept of care and strives to improve the quality of life, dignity and comfort of the patient and family at end of life.
Hospice teams consist of nurses, social workers, chaplains, and volunteers. The role of hospice is to address medical,
nursing, psychological, social and spiritual needs for both the patient and family. Hospice is covered by most health
insurances, however does not provide 24 hour direct caregiving to patients.

Housing: Assisted Living Facilities
Assisted Living Facilities are for individuals who are fairly independent but require some daily supervision and assistance
with house chores and personal care. Assisted living facilities may offer rooms or apartment-style accommodations and,

Housing: Skilled Nursing Facilities (SNF)
Skilled Nursing Facilities (SNF) offer 24 hour personal and medical nursing care to residents. SNFs can offer short term
rehabilitation services for patients just released from an inpatient hospital, such as physical therapy, occupational
therapy, speech therapy, medication management, IV feedings, etc., as well as long term care for patients that are no
longer able to live in an independent environment. The facilities listed below were provided as possibly having some

In-Home Assistance
In-home assistance consists of companionship, housekeeping, cooking, personal care (dressing, grooming, bathing, etc),
and grocery shopping. This service is usually provided on an hourly basis and is not covered by most health insurances.
(Medi-Cal patients can apply for In Home Support Services which provides the above services.) Most in home assistance

When seeking medical equipment, remember to consult your doctor for appropriateness and safety. Some medical

Local Multipurpose Senior Service Program (MSSP) sites
