ARRIVAL & PARKING: The UC Davis Spine Clinic is located at,
3301 C Street, Suite 1500, Sacramento, CA 95816. Please
see the map on the back page for directions. Parking is
available in front of the Spine Center at no charge.
BILLING INFORMATION: Our Registration staff will need to
photocopy your medical insurance information. Please provide
them with:
• An insurance identification card and any co-payment
amounts required by your insurance company
• A referral form from your referring physician (if applicable)
• A Medicare card, if Medicare is your insurance
• The name, telephone number, and address of your emergency
contact person
• The name of the person financially responsible for your
medical bills
Please Also Remember to Bring the Following
X-rays, MRIs, Bone Scans, EMG results, and all other important
medical records pertaining to your diagnosis

Under SPECIALTIES in the left hand column, choose the
service you will be seeing and find the patient
questionnaire on that page. Please complete it
and bring it with you to your appointment.
In order for your physician to properly diagnose and treat your
medical condition, there may be the need to obtain x-rays.
This can add additional time to your visit. The average visit to
UC Davis Orthopaedics is approximately 2 hours. Please plan
to arrive 15 minutes prior to your appointment time to
finalize your paperwork.
What is a teaching hospital?
A teaching hospital or academic medical center is a hospital
that is associated with a major medical school such as UC Davis.
Teaching hospitals train medical students, nursing students,
residents and others to become skilled medical professionals.
When you come to UC Davis Orthopaedics for an appointment,
you may be examined initially by a resident. After the
examination, the resident will present the clinical data to
the attending physician, who will then examine you and
review the studies available. Your care will always be anchored
by one of our many experienced, board-certified physicians.
From the North (Redding, Woodland, Sacramento International Airport)
Take I-5 South to 50 East/ I-80 Business Route East (South Lake Tahoe). Take the I-80 Business Route/Capital City Freeway towards Reno. Take the
“H” Street exit. Turn Right onto “H” Street. Turn Left on 33rd Street.
From the Northeast (Reno, Roseville, Auburn)
Take I-80 West (Sacramento). Merge onto Capital City Freeway (Right lanes)
Take the “E” Street exit. Make slight Left onto 29th Street. Turn Left on
“E” Street. Turn Left on 33rd Street.
From the West (Davis, San Francisco)
Take I-80 East (Sacramento). I-80 becomes Highway 50 East. Take the I-80
Business Route/Highway 99 South exit. Stay in Left lane onto I-80 Business
Route/Capital City Freeway East (Reno). Take the “H” Street exit. Make a
Right onto “H” Street. Turn Left on 33rd Street.
From the East (Placerville)
Take Highway 50 West. Take the I-80 Business Route/Capital City Freeway
exit. Take the “H” Street exit. Turn Right onto “H” Street. Turn Left on
33rd Street.
From the South (Stockton, Fresno)
Take Highway 99 North. Merge onto I-80 Business Route/Capital City Freeway (Reno). Take the “H” Street exit. Turn Right onto “H” Street. Turn Left
on 33rd Street.
