ב"ה MICHIGAN JEWISH INSTITUTE Policy and Procedure Manual Functional Area: Title: RECORDS MANAGEMENT Policy No. RM 1106 Completing Add/Drop and Drop Forms Effective Date: June 2002 Page No. 1 of 3 GENERAL PURPOSE: This policy is intended to provide a process and procedure that will insure the correct completion of Add/Drop and Drop forms by students and the correct entry of same into students’ electronic and hard copy records of course registrations and grade records. POLICY: It is the policy of MJI that appropriate forms be used at all times and that there be 100% consistency in the use of all course numbers and names. The identification of course numbers and names on Add/Drop and Drop forms and their manner of entry into student records (electronic and hard copy) should be identical to those listed in the current MJI Academic Catalog All MJI employees responsible for student Add/Drop and Drop Forms and/or data input into student records will be oriented on this policy and procedure and will make it part of their records management activities. PROCESS: To insure that all Add/Drop and Drop forms are accurately completed by students and entered accurately into MJI electronic and paper files, the following procedures are to be used: General Procedures 1. MJI staff are not authorized to accept any Add/Drop or Drop forms from students that are incomplete. Therefore, staff should audit these forms as students turn them in. Should a student choose to mail an Add/Drop or Drop form to MJI or otherwise submit it without immediate and direct staff intervention, upon determination that a so delivered form is incomplete, it is to be immediately returned to the student via mail or other means for correction. Data entry of information taken from properly completed Add/Drop and Drop forms is to be conducted only by MJI staff appropriately trained for this activity. Due professional diligence is expected so that all related grade records, transcripts, financial information and other documents will contain consistent course numbers, names and registration information. Add/Drop Forms 3. The official Add/Drop period begins on the first day of instruction for a particular course and continues through the next six days. Students will not be permitted to add courses after the first week they are in session without special permission from the Dean of Academic Administration. 4. Courses added by students subsequent to a timely initial registration of other courses will be charged at standard tuition rates. No additional registration fees will be charged for added courses. Students are responsible for all additional tuitions and fees that may be required for the added courses. 5. During the first week of the semester (the Add/Drop period), courses may be dropped without any reference of the courses dropped recorded on the student’s permanent record. Drop Forms 6. Thereafter, students may officially withdraw from a class using the Drop form, before the end of the fourteenth week of the semester. Students dropping courses after the first week will receive a mark of “WP” if they were passing when they withdrew, or a “WF” if they were failing. Neither grade will count towards the Grade Point Average. A student who withdraws after the end of the fourteenth week will receive a failing grade of “F” which will count towards the Grade Point Average. Dropping of courses after the first week of a semester has implications for both students receiving financial aid and for private pay students. The MJI Office of Financial Aid must receive a copy of all Drop forms so that proper adjudication of students’ accounts may be conducted. This policy is effective immediately upon its authorization and publication. Model Add/Drop and Drop forms have been attached to this policy for use as needed.
