Permissions will be assigned for the person to request access to services in the department and any sub-units of the department. If this is not required, then please specify in Section 2. Name: Signature: Guidance for completing the Services System registration form: Section 1: Only staff with a valid, current UCL user ID can be registered for the Services System. If in doubt, please contact your departmental computer representative or Information Systems User Services. Section 2: The Departmental Administrator role gives full access to information for the selected department. This includes requesting, reviewing and revoking all types of service (computer accounts, telephone registrations, library access, ID cards and building access). It also allows for the creation of new Visitor records. Users with the DA role will often be key holder signatories for the department. The Computer Rep role only allows for the creation and maintenance of requests for UCL computing resources. The Telecoms Rep role only allows for the creation and maintenance of telephone registration requests. Departments: only departments and sub-units recorded in the HR organisation structure can be used to restrict or allow access to the Services System. Section 3: This section should be signed by your Head of Department. ISD can only process requests on receipt of a signed copy of the registration form. Section 4: This section should only be completed by the Services System service owner in ISD.
