JOB ACCOUNTABILITIES: *E/M/NA % TIME ______ ______ Provides drug therapy consultations and on-site drug information to medical, nursing, or other hospital staff. ______ ______ Coordinates and teaches a required clerkship for pharmacy students. ______ ______ Provides education in the patient care environment, small group discussions and classroom. ______ ______ Serves as a role model practitioner, demonstrating the integration of pharmaceutical and medical sciences toward the improvement of patient care outcomes. ______ ______ Participates in the selection of appropriate medications for specific indications. Maintains records on controlled drugs. Assists nurses with routine updating of patient Medication Administration Record (MAR) as appropriate. ______ ______ Uses interviews, physical assessment skills and interpretation of laboratory parameters to monitor for adverse and therapeutic effects. ______ ______ Oversees medication schedules in relation to each patient's ability to metabolize and eliminate specific drug compounds, e.g.. aminoglycoside kinetics. Registered pharmacist in state of California. Hospital pharmacy experience. Familiarity with unit dose, IV admixtures and clinical pharmacy practice. Preferred Field of Expertise: Completion of residency program or fellowship training. Hospital pharmacy experience.
