IDEA forms Process Fall 2015 1) Form Distribution and request format (note new Senate Office location) 2) Online Evaluations (for online courses and face-to-face courses) 3) Additional Information and resources 4) Calendar for Fall 2015 ______________________________________________________________________________ 1) FORM DISTRIBUTION The Faculty Information Forms (FIF), Student Rating Forms (SRF), and Diagnostic Forms can now be requested for pick up. To request your forms: Email me at venitab@csufresno.edu with the following information: 1) Department name 2) Approx number of Faculty Information Forms (FIFs) needed for Fall (one for each course to be evaluated) 3) Approx number of Student Rating forms (Short Forms) needed for Fall (one for each student enrolled in the courses to be evaluated) 4) Approx number of Diagnostic Forms needed for Fall (one for each student enrolled in the courses to be evaluated) 5) Any special notes or needs regarding the pickup. Please wait to receive email confirmation before coming to the Senate office to pick your forms. The confirmation email will establish a pick up time. Note: The Senate Office is now located in the Henry Madden Library, room 3206 (3rd floor in the south wing). Sample Email for IDEA forms request: Department of Learning Faculty Information Forms: 30 Short Forms: 1,200 Diagnostic Forms (If needed): 200 2) ONLINE COURSES After you have collected the courses that will be evaluated for your department, in a separate email, send courses to be evaluated online directly to Jolynne Blake (jblake@csufresno.edu). Include venitab@csufresno.edu as a "cc". Remember this includes online courses as well as face-to-face courses. Once received, TILT will set up the online module for the IDEA Center via Blackboard. The deadline to submit courses to be evaluated online is no later than October 16, 2015 When emailing your online courses, make sure "online courses and your department" appear in the subject line. Also indicate in your email whether the courses are fully online or face-to-face. Use the following format: COURSE# 33444 Subject BUS 101 Last Name (Instructor) Smith First Name (Instructor) Mary PLEASE DISCOURAGE FACULTY FROM CONTACTING TILT DIRECTLY. 3) ADDITIONAL INFORMATION RESOURCES (Local Codes) As a reminder, the Office of Faculty Affairs continues to update the APS "What’s New" website with links and information regarding the process. This is permissible; however, the deadline to submit online course evaluations (October 16) and the deadline to bring evaluations to the Senate Office (December 15) are firm deadlines and are not flexible.
