Background The intersection of County Street (Route 140)/Mozzone Boulevard in Taunton, Massachusetts, was listed as a High Crash Location within the Southeast Regional Planning and Economic Development District (SRPEDD). The Massachusetts Department of Transportation (MassDOT) has determined that Highway Safety Improvement Program (HSIP) funding would be allowed for reconstruction of the intersection if a Road Safety Audit (RSA) were conducted and a proposed design incorporated the safety improvements identified in the RSA. The intersection was identified as one of 19 signalized High Crash Locations that would receive funding for signal improvements as part of the Way Forward program. Two projects are currently in the MassDOT design phase that will impact the intersection of County Street (Route 140)/Mozzone Boulevard. MassDOT Project #606729 will widen County Street at the railroad crossing approximately 650 feet south of Mozzone Boulevard to eliminate an existing bottleneck at the crossing. This project is on the Transportation Improvement Plan (TIP) for construction beginning in spring 2015. A project that will resurface the length of County Street (MassDOT Project # 605171) is currently ongoing, and is expected to be completed by the spring of 2015. In general, an RSA is intended to identify potential safety improvements that can be evaluated and included as part of the design process for a future reconstruction project. The short-term, low-cost potential improvements could be considered by the responsible agency for immediate implementation prior to reconstruction, as appropriate. The RSA study area is illustrated in Figure 1. Project Data The audit team conducted an RSA for the intersection of County Street (Route 140)/Mozzone Boulevard on Wednesday, February 19, 2014. The RSA agenda appears in Appendix A. Table 1 lists the audit team members and their affiliations. Appendix B provides contact information for all team members. From 2009-2012, 18 crashes were reported at the intersection. Of these crashes, 10 (or 56%) were rear-end crashes, four (21%) were angle crashes, two (11%) single-vehicle crashes, one (6%) was a head-on crash, and one (6%) was a sideswipe crash. Of the 18 crashes, 10 (56%) occurred during daylight hours, 14 (78%) occurred during clear weather, and 16 (89%) occurred on dry pavement. Forty percent of motorists involved in crashes were between the ages of 15-29, possibly due to the nearby Bristol-Plymouth Regional Technical School and retail destinations. Of the 18 crashes, five (28%) resulted in personal injury. Public safety officials attending the RSA stated that the actual number of crashes at the intersection is likely higher than the 18 reported due to technical issues with crash submission to the Registry of Motor Vehicles and discrepancies with the location of several crashes. It was also stated that a significant number of low-severity crashes occur, where operator’s reports are filed but no police record is created. Project Location Description The RSA focused on the intersection of County Street (Route 140)/Mozzone Boulevard in the City of Taunton, as shown in the aerial image in Figure 2. County Street (Route 140)/Mozzone Boulevard is a signalized intersection with three approaches. The Mozzone Boulevard eastbound approach consists of a left-turn lane and a right-turn lane. The County Street (Route 140) northbound and southbound approaches each consist of two general purpose travel lanes with no lane designations. There are no sidewalks or crosswalks provided at the intersection, and the provided shoulders are too narrow to be considered an adequate bicycle accommodation. The County Street northbound approach is provided with a short protected left-turn phase at the beginning of the cycle; however, on the day of the audit, team members reported that this left-turn phase was sometimes not called. Left turns are permissive after this short protected phase. If and when this phase is provided, the Mozzone Boulevard eastbound right-turn movement receives an overlap phase. This intersection is the junction of the following roadways, which are categorized according to MassDOT Office of Transportation Planning functional classifications: Mozzone Boulevard is a local roadway and falls under the jurisdiction of the City of Taunton. Mozzone Boulevard runs northeast-southwest between the Route 24 Water Protection Facility to the southwest and County Street at the study area intersection to the northeast. Mozzone Boulevard consists of one travel lane in each direction. Sidewalks are not provided along the roadway. County Street (Route 140) is classified as an urban minor arterial within the study area, and falls under MassDOT’s jurisdiction. County Street runs northwest-southeast, originating from Route 12 in Winchendon to the northwest and terminating at Route 6 in Fall River to the southeast. Within the study area, County Street consists of two travel lanes in each direction; however, at a railroad crossing approximately 650 feet southeast of the intersection, County Street narrows to one lane in each direction before for a brief distance before widening to two lanes in each direction southeast of the crossing. Pedestrian and bicycle accommodations are not provided on County Street in the study area. The speed limit on County Street within the study area is 40 mph. Road Safety Audit Observations and Potential Enhancements Based on discussions on Wednesday, February 19, 2014, the RSA team determined that the intersection of County Street (Route 140)/Mozzone Boulevard has the following issues that affect safety: 1. 2. 3. 4. The following sections describe in more detail the safety issues and enhancements determined during the RSA. Several of these issues require further study and engineering judgment to determine the feasibility of implementing enhancements to address them. Traffic Signal Observations: Nine of the 18 crashes that occurred at the intersection were rear-end crashes between County Street northbound vehicles. County Street consists of two lanes in each direction, with no dedicated turning lanes. Rear-end crashes may occur when a County Street vehicle unexpectedly slows or stops to take a left onto Mozzone Boulevard. The County Street northbound approach receives a brief advance protected left-turn phase before County Street southbound vehicles receive a green County Street northbound vehicles receive a leading protected left-turn light. However, this protected phase was not phase. functioning properly during the RSA and was not provided in every signal cycle. Motorists wishing to turn left onto Mozzone Boulevard without the green arrow must wait for a gap in two lanes of County Street southbound traffic. Three angle crashes occurred between County Street southbound through vehicles and County Street northbound vehicles turning left onto Mozzone Boulevard. Rear-end crashes may occur when a southbound motorist sees opposing traffic moving and assumes the southbound signal has turned green as well. One rear-end crash occurred between County Street southbound vehicles. RSA team members noted that there is currently no preemption between the signal at County Street/Mozzone Boulevard and the railroad crossing southeast of the intersection. However, the signal is coordinated with the signal at County Street/Route 24 off-ramp. Rear-end crashes may also occur if the clearance interval at a signal is too short. RSA team members stated that clearance intervals appear to be adequate. Eight of the 18 crashes at the intersection occurred at night. RSA team members stated that the intersection has sufficient lighting. Backplates are installed on the signal heads; however, they are not retroreflective. During the RSA, team members observed a vehicle stopped at the stop line in the Mozzone Boulevard eastbound left-turn lane. Mozzone Boulevard eastbound did not receive a green indication for several minutes, and only received a green indication when another vehicle approached the intersection in the right-turn lane. This issue did not occur again during the RSA. RSA team members noted that power lines partially obstruct the Mozzone Boulevard eastbound signal indications. Confirm that clearance intervals are sufficient at the intersection, which could reduce the occurrence of rear-end crashes. 2. Install retroreflective backplates on signal indications to increase visibility of the signals. 3. As part of long-term planning efforts, provide preemption between the railroad crossing southeast of the intersection and the intersection. 4. Verify that the vehicle detection for the Mozzone Boulevard eastbound approach and the County Street northbound approach is fully functional and covers the appropriate width of the roadway. 5. As part of long-term reconstruction efforts, place signal indications so that they are not obscured by power lines. Intersection and Approach Geometry Observations: County Street generally has two travel lanes in each direction; however, the cross-section narrows to one wide travel lane in each direction at the railroad crossing approximately 650 feet southeast of Mozzone Boulevard before widening to two lanes in each direction southeast of the railroad crossing. RSA team members reported that the lane drop impacts motorist behavior at the intersection. The lack of a dedicated left-turn lane may also contribute to angle crashes, as left-turning motorists may accept a smaller gap in opposing traffic if they perceive pressure from vehicles waiting behind them. Evaluate the safety benefits of providing an exclusive left-turn lane on the County Street northbound approach by designating the existing general purpose inside lane as an exclusive left-turn lane to reduce the number of rear-end crashes at the intersection. Consider impacts to intersection operations and storage length of the remaining through lane, particularly in regards to the railroad crossing southeast of the intersection. Consider potential new traffic at the intersection as a result of potential development. 2. As part of long-term reconstruction efforts, consider widening the roadway to provide an exclusive left-turn lane on the County Street northbound approach while maintaining two northbound through lanes. 3. Relieve the bottleneck southeast of the intersection by widening the roadway to a consistent 4-lane cross-section in the vicinity of the railroad crossing. 4. If an exclusive left-turn lane is provided, install flashing yellow arrow functionality for the County Street northbound left-turn signal. Studies have shown that a flashing yellow arrow during a left-turn permissive phase can reduce the number of left-turn angle crashes. Signage and Pavement Markings Observations: There is no lane designation signage approaching the intersection to indicate to motorists that the travel lanes are general purpose lanes. Motorists using the inside travel lane on County Street northbound may not expect a vehicle to be stopped in the lane waiting to turn left. Street name signage, while provided, is not immediately visible to motorists, particularly on the County Street southbound approach to the intersection. There is no stop bar provided on the Mozzone Boulevard eastbound approach. Enhancements: 1. Place lane designation signage along County Street indicating that the inside lane on the County Street northbound approach is a shared left-turn/through lane. Place similar signage along the County Street southbound approach indicating the outside lane is a shared through/right-turn lane. 2. Place street name signage on mast arm shafts to increase visibility. 3. Provide advance signage along County Road reading “Mozzone Boulevard Next Signal” to increase awareness of the intersection and aid wayfinding. 4. Remove signs if they are in the SHLO to improve motorist attentiveness and visibility on County Street. 5. Provide a stop bar on Mozzone Boulevard. 6. Safety Issue #4. Pedestrian and Bicycle Accommodations Observations: There are no pedestrian accommodations at or near the intersection. RSA team members stated that pedestrian traffic is very limited. On the day of the audit, a pedestrian was seen walking in the road on County Street southbound and Mozzone Boulevard eastbound because of snow cover on the edge of the roadway. There are also no dedicated bicycle accommodations at or near the intersection. Shoulders are provided on County Street; Sidewalks, crosswalks, pedestrian signals, however, they are not wide enough to be and bicycle accommodations are not considered a bicycle accommodation. Place “Pedestrians and Bicycles Restricted” signage between Industrial Drive and the Route 24 southbound on-ramp to alert pedestrians and bicyclists that Route 140 is restricted to vehicles only south of Route 24. Recommendations Based on its review of data, on-site field observations and group discussion, the RSA team identified the following possible enhancements that could improve safety at the intersection of County Street (Route 140)/Mozzone Boulevard. Further study and design work will need to be conducted to determine the feasibility of making some of the improvements. Table 2 summarizes the safety issues, possible enhancements, estimated safety payoff, time frame, cost, and responsibility. Safety payoff estimates are based on engineering judgment and are categorized as low, medium, and high. The time frame is categorized as short-term (<1 year), mid­ term (1 to 3 years), or long-term (typically >3 years). Long-term improvements are typically considered to be substantial improvements with an expected time frame for implementation greater than 3 years. The costs are categorized as low (<$10,000), medium ($10,001 to $50,000), or high (>$50,000). Short-term enhancements include, but are not limited to: Install retroreflective backplates on signal indications; Provide lane use signage; Provide street name signage; Remove unnecessary signs from the SHLO; and Restripe pavement markings on Mozzone Boulevard. To enhance the safety of the corridor, the long-term enhancements are to: Remove the bottleneck on County Street at the railroad crossing by providing four travel lanes; Provide a dedicated left-turn lane on County Street northbound; Install a sidewalk along County Street; and Provide a shoulder for bicyclist use on County Street. The RSA is intended to identify potential safety improvements that can be evaluated and included as part of the design process for a reconstruction project. Vh3 fled the scene. 54 Vh1 failed to yield right of way to Vh2 and Vh3 Opr of Vh1 stated that an oncoming Vh crossed over into his lane and ran him off the road into utility pole #511 Vh2 failed to stop in time at the signal and rear ended Vh1. Vh1 had stopped for red light. Vh2 rear ended Vh1 and fled the scene. Vh1 rear ended Vh2 which was stopped in traffic and the impact caused Vh2 17 to strike Vh3. Opr of Vh1 stated that his foot slipped off the brake pedal and that caused Vh1 to rear end Vh2. The impact also Vh2 to rear end Vh3 which standing 27 ahead. Vh1 rear ended Vh2 which was stopped in traffic and the impact caused Vh2 24 to strike Vh3. Vh1 was at fault for not yielding right of way to Vh2. Vh1 was in blind spot of Vh2 and both the Vh's collided when Vh2 tried to change lanes. An unknown SUV cut off Vh1 and Vh1 crashed into Olive Garden rock wall. Vh1 failed to yield right of way to Vh2 and both the vehicles collided. Vh1 followed too closely and couldn't stop in time when Vh2 stopped. Vh1 failed to stop in time and rear ended Vh2. The impact pushed Vh2 to 29 rear end Vh3.
