To apply for a waiver of a required second summer course, please complete this form and return it to
the Office of Academic Support Services by the above due date. Documentation such as course
syllabus, outlines, bibliographies, etc. is required for your waiver to be considered. Your transcripts
documenting prior course work as the basis for this petition should already be on file. If they are not, we
will contact you via your Smith email account. The Registrar will communicate all petition decisions via
Smith email.
