You will need to request a new I-20 Žƌ^ͲϮϬϭϵwhen you change your level
(BS, MS, PhD) or Major/Program. This form can be emailed to
International Programs and Services (IPS).

You MUST attach the following documents if you are CHANGING or ADDING MAJORS or MINORS
Undergrads: You must first complete the “Change/Addition of Major or Minor” form from the
Registrar’s office.
Graduate students: You must attach a copy of your acceptance letter from the Graduate School
indicating your new department/major.

If you are advancing from BS to Graduate (MS or Phd), you must attach the following documents:
1.
Copy of your acceptance letter from the Graduate School.
2.
Copy of your financial guarantee, bank statement, or offer letter from your department
showing enough funds to cover at least 1 academic year and a completed Certification
of Finances Form.
