EARLY MEDICAL SCHOOL ACCEPTANCE
APPLICATION INFORMATION
Thanks for taking the time to complete your Sophomore Assessment Meeting with a premedical
adviser at Xavier University of Louisiana, during which time your interest in applying to early
medical school acceptance programs through Saint Louis U, U of Rochester, Dartmouth, and/or
Tulane U was discussed. NOTE: If you are viewing this document and have NOT met with
a premedical adviser to discuss program eligibility, you must sign-up for and complete the
Sophomore Assessment Meeting ASAP. Only students who complete the One-on-One
Sophomore Assessment Meeting with Ms. QV during the allotted time period will be able to
proceed with the application process!!!
The attached SAMPLE application should be used as a GUIDE to complete the blank application
for the early acceptance program(s) that you are applying to.
You should view the individual program requirements that you picked-up in preparation for your
Sophomore Assessment Meeting for SPECIFIC application information; you should also
complete the tasks outlined below by the specified dates…

By February 15th, submit your medical school-ready Personal Statement and PSE Form to PM Office for
review at xupremed@yahoo.com and CONTINUOUSLY revise and submit accordingly BEFORE giving
them to faculty to help write your letters of evaluation and before copying and pasting them to your draft
early acceptance application (both documents should be in MS Word---.doc or .docx extension---and
renamed PSLastNameFirstName and PSELastNameFirstName);
By February 15th, ask faculty members if they would be willing to write “medical school”---not schoolspecific--- letters of evaluation on your behalf (2 BCPM teachers and 1 non-BCPM teacher);
By March 1st, submit the 1) Faculty Evaluation Cover Letter, 2) Revised Personal Statement, and 3) Revised
Post-Secondary Experiences Form to EACH faculty member who agreed to write a “medical school”
evaluation on your behalf (do this no later than March 1st to give teachers time to write the letters)---you
should indicate on the Faculty Evaluation Cover Letter that the letter should be submitted to the PM Office
by April 15th;
By March 15th (i.e. AFTER you have submitted your PS and PSE Form at least TWICE during the
2015-2016 academic year), complete one (1) draft application for early acceptance in its ENTIRETY and
send to xupremed@yahoo.com for review by Ms. QV. Since all four (4) early acceptance applications are
essentially the same, you need only submit one (1) draft application for review if f you are applying to
multiple programs. FYI, go to www.xula.edu/premed and click on “XU Students Interested in Medicine,
Dentistry, etc.”, then scroll down to the individual early acceptance program links to access each BLANK
program-specific application. You may submit your draft application to Ms. QV at xupremed@yahoo.com as
many times as needed through April 30th. IF YOU HAVE NOT SUBMITTED YOUR PS and PSE FORM
TO THE PREMED OFFICE FOR REVIEW AT LEAST TWO TIMES DURING THE 2015-2016
ACADEMIC YEAR, YOUR DRAFT APPLICATION WILL NOT BE ACCEPTED AND REVIEWED BY
MS. QV!!!;
By March 15th, request to have an OFFICIAL ACT or SAT score report sent directly to you (if you are
applying to SLU) and then bring the official score report to the Premedical Office for filing;
By March 15th, request to have official transcripts from OTHER colleges or universities you attended sent
directly to the Premedical Office (you need one transcript for EACH of the programs you’re applying to);
By April 30th, submit your LAST DRAFT application to xupremed@yahoo.com for FINAL review by Ms.
QV. Any draft applications received after April 30th will NOT be reviewed; they will be considered as
FINAL submissions to be reviewed for nomination AND then forwarded to the medical schools IF YOU
ARE NOMINATED!!!;
As soon as you take your last FINAL EXAM in the Spring, go to XU’s Registrar’s Office to request that
an official transcript be sent directly to the Premedical Office (you need one transcript for EACH of the
programs you’re applying to; be sure to check “Hold for Final Grades” if final grades have NOT been posted
at the time you make your request); and
By May 15th, print, sign, and submit (either in-person, by mail, or by email) your FINAL application (one
per program—make sure you have the CORRECT application for each program—header and final page is
program-specific) to the Premedical Office. Be sure to include your FINAL grades on the application.

Again, make sure that your evaluations, official transcripts from all colleges/universities attended,
and ACT/SAT Score Report (SLU) are on file in the PM Office NO LATER THAN May 15th.
Go to http://www.actstudent.org/scores/send/ for the ACT Score Report and
http://sat.collegeboard.org/scores/ for the SAT Score Report. In the past, early acceptance
applicants have encountered problems with getting official score reports sent directly to the
Premedical Office. As such, we ask that you request to have an official copy sent to YOU and
then you may bring it to the Premedical Office for filing.
Official transcripts, letters of evaluation, and FINAL
applications may be mailed to us directly at: Premedical
Office, Xavier University of Louisiana, 1 Drexel Drive,
Box 120, New Orleans, LA 70125.

COMPLETING ALL SECTIONS OF THE APPLICATION!!!
Coursework Section Notations/Instructions (see sample attached):
NOTE: YOU MUST INCLUDE ALL OF THE COURSES THAT YOU HAVE TAKEN AND
ANTICIPATE TAKING IN ORDER TO GRADUTE FROM XAVIER IN THE
COURSEWORK SECTION---AND YOUR APPLICATION MUST INCLUDE YOUR SPRING
2016 SEMESTER GRADES!!!
The coursework section should ONLY take up TWO (2) pages on the application. Be advised that if
you use more than one line when entering a particular course (e.g. lengthy course names), it may force the
pre-formatted document to take up more than two pages for the coursework section. Since the document
should ONLY take up TWO (2) pages, you'll have to be mindful of this when completing this section of the
application.
To avoid this issue, you should STOP entering courses on PAGE ONE if you find that rows from PAGE
ONE are being added to the next page, thereby creating an additional, unnecessary page for the
"Coursework" section. If this happens, simply delete the text you entered that forced an additional page to
be added for the "extra rows," and then begin entering your remaining coursework on PAGE TWO of the
"Coursework" section. Again, it should look like the sample that you received during your sophomore
assessment with Ms. QV.
*Academic Status = FR, SO, JR, SR (put BLANK row to separate academic statuses, see sample)
*BCPM/AO= indicate whether the course is Biology, Chemistry, Physics, Math, or Any Other (non BCPM)
*Academic Year = each academic year begins with the summer session (if you attended summer school) and ends with
the spring semester. For example, if your Freshman year at XU began with you attending summer school during
summer 2014, then the academic year for Freshman year would be 2014 with terms including SS (summer session), S1
(Fall 2014), and S2 (Spring 2015)
*Term=SS (Summer Session); S1 (Fall Semester); and S2 (Spring Semester)
*Course Name = as it appears on your transcript
*Number = as it appears on your transcript
*Type = H (Honors class); AP (courses for which you received AP credit OR that you bypassed by examination or test
scores; for example, if you received PC for a course, it should be entered as “AP); CC (courses that you are currently
enrolled in OR anticipate taking in the future)
*Grade = as it appears on your transcript
*Semester hours= as appears on your transcript
-Use sample coursework section as a guide for data entry.
-NOTE about AP Credit from AP Exam: Include AP credit granted by the College Board AP Program in the
coursework section under the same semester/year that it appears on your Xavier Transcript. However, it should be
entered at the end of the coursework section (See sample).
