Jefferson Community Healthcare Center, Inc.
Clinical pharmacy services inclusive of, but not limited to, the following disease state management services: diabetes
education and management, hypertension education and management, cholesterol education and management, and
obesity education.
Xavier University Health & Wellness Center
Pharmacist-directed, freestanding health center. Services include diabetes education and management.
Interim LSU Public Hospital- Coumadin Clinic/ STITCH Clinic
This ambulatory medicine rotation will introduce the resident to the responsibilities, activities, and practice standards
associated with a clinical pharmacist in an outpatient setting. Residents will provide patient care and other
pharmaceutical services to the patients and their families to ensure optimal clinical outcomes. Residents will also
develop skills in the management and usage of resources in the LSU healthcare system to improve therapeutic
outcomes associated with medication usage. Emphasis will be placed on the role of the pharmacist as a member of a
multidisciplinary health care team. Clinical pharmacy services include, but are not limited to patient education &
counseling, and medication therapy management (MTM) of hypertension, smoking cessation, diabetes, dyslipidemia,
metabolic syndrome, hypercoagulable states, cardiac valve replacements, and venous thromboembolism. The
resident will also collaborate with the healthcare team to formulate a pharmaceutical care plan for each patient and
document that information in the patient’s chart.
Tulane Community Health Clinic at Covenant House
Tulane Community Health Clinic at Covenant House provides primary care to those with limited access. They also
have extensive behavioral health services and provide social services as well. The clinic aims to provide health care
as part of a medical home involving physicians, a nurse practitioner, a clinical pharmacist, medical assistants, and
community health workers. The clinical pharmacy service provides medication titration, nutrition counseling, smoking
cessation counseling, and health maintenance services for patients with uncontrolled diabetes and hypertension. In
addition to direct patient interaction, a telemedicine clinic monitors patients with uncontrolled diabetes needing more
intense counseling.
Internal Medicine Clinical pharmacy services include: participation in daily rounds with Internal Medicine Team,
which involves round table discussions, and seeing & following patients on service; providing complete
pharmaceutical care to the team by assessing, monitoring, and recommending appropriate medication therapy to our
patients based on available medical evidence; providing pharmacokinetic dosing, monitoring of toxicities, reporting
ADRs, providing medication counseling, communicating information to other healthcare providers (physicians,
pharmacists, nurses, and other ancillary members) as necessary, and participating in ongoing Drug Utilization
Reviews (DUEs); and providing drug information education.

Xavier/Rite Aid Pharmacy Care Center
Pharmacist-directed disease state management center in a community/retail setting. Target disease states for MTMS
services include but are not limited to diabetes and cardiovascular disease. This practice site has a large community
service component the majority of which is currently conducted at a local assisted living facility and senior centers
Tulane-Xavier Drug Information Center
The Xavier University of Louisiana College of Pharmacy Drug Information Center is affiliated with Tulane University
Hospital and Clinic and is established to provide accurate, current, and unbiased drug information services to area
health care practitioners
Interim LSU Public Hospital - HIV HOP Clinic
Consultant clinical pharmacy services in HIV/AIDS care to providers and patients on an on-going basis. Practice
involves patient and provider education, individual and group clinical consultations, telemedicine series to
providers, individual and group traineeships, development of clinical materials in response to questions and hands-on
problem-solving with medication-related issues in the complex process of HIV/AIDS disease management.
Ochsner Clinic Foundation Asthma Education Center
The Asthma Education Center is a service that provides asthma education, instruction on proper use of asthma
devices, and peak flow monitoring to patients. Our goals include enhancing asthma care, patient knowledge, and
patient satisfaction with care through collaboration with physicians pharmacists, patients, and families. Residents will
aid the provider in developing asthma action plans, educating patient on asthma, and teaching proper use of asthma
devices such as spacer and peak flow meters .Residents will also be exposed to other conditions seen in clinic,
including rhinitis, chronic urticaria, chronic cough, and drug and food allergy.
Tulane University Hospital Clinic
The clinic provides primary care for HIV-infected patients; consult services for infectious diseases, and infectious
disease clearance for renal and liver transplant patients infected with HIV. The resident will be working with the
preceptor and other healthcare professionals (physicians and nurses) to provide services to the clinic patients. In
addition, the resident will be responsible for identifying, preventing and resolving medication therapy issues as well
as providing medication/adherence counseling for patients infected with HIV, and serving as a drug information
resource.
Daughters of Charity Services New Orleans
The clinical pharmacy services provided include patient counseling, preventative services, drug information services,
medication reconciliation, disease state management, medication monitoring, and provider consultation. The primary
focus of these services includes the interdisciplinary management of cardiovascular diseases, such as
anticoagulation and dyslipidemia. The services are provided in a community healthcare center that operates based
on the medical home model. Additionally, a 340 B outpatient pharmacy, mobile unit, dental clinic, and behavioral
health professionals are located on-site to enhance the availability of the services provided.

Interim LSU Public Hospital – Stroke & Heart Attack Prevention (SHAP) Clinic;
Obesity Weight Loss and Management Bariatric (OWL MBA) Clinic
This ambulatory medicine rotation will introduce the resident to the responsibilities, activities, and practice standards
associated with a clinical pharmacist in an outpatient setting. Residents will provide patient care and other
pharmaceutical services to the patients and their families to ensure optimal clinical outcomes. Residents will also
develop skills in the management and usage of resources in the LSU healthcare system to improve therapeutic
outcomes associated with medication usage. Emphasis will be placed on the role of the pharmacist as a member of a
multidisciplinary health care team. Clinical pharmacy services include, but are not limited to, patient education &
counseling, and medication therapy management (MTM) of hypertension, diabetes, dyslipidemia, thyroid disorders,
cardiovascular disease, metabolic syndrome, and other endocrine disorders.
Interim LSU Public Hospital - Emergency Medicine
During this core rotation learning experience, the resident will spend 6 weeks at the LSU Interim Health Sciences
Trauma I Center emergency room in critical and non-critical areas (i.e. Pelican and Gulf pods). The practice
environment in the ED Critical will require moment-to-moment interactions with the ED attending staff (Consultants),
ED Residents (and rotating non-ED house staff), mid-level practitioners, nursing staff, pharmacy staff and other
healthcare professionals in the work-up of drug-related problems and in optimizing pharmacotherapy for all ED.
Critical patients. In addition to monitoring and consulting on emergency pharmacologic issues in medical and trauma
patients, the resident will participate in all ED Pharmacy Practice model activities including active roles in response
Code Blue, Level 1 Trauma resuscitations, Stroke Team activities, ED Sepsis Response and patients undergoing
Procedural Sedation. The resident will also provide information education to nursing, pharmacy, physician staff
and patients/ family members. Documentation of pharmacy related interventions and activities will done in Quantifi
and Cliq™ (as appropriate).
Children’s Hospital of New Orleans- Pediatric Critical CareResidents will have the experience of providing pharmaceutical care to patients in the Pediatric Intensive Care Unit
including but not limited to pharmacokinetics, nutrition, infectious diseases, sedation, and cardiology. As part of a
multi-disciplinary team, residents will be responsible for ongoing team education and patient management.
East Jefferson General Hospital- Inpatient OncologyClinical pharmacy services provided inclusive of, but not limited to, medication therapy management (MTM) of
hematology/oncology patients.
