As part of University of Puget Sound's 125th anniversary in 2013, all members of the Logger community are invited to create compelling short films that imaginatively celebrate the spirit of Puget Sound. Please complete, sign, and submit this form with your contest entry.

Contest entrants must gain consent from all individuals whose name, voice, image, and/or likeness is identifiable in their video submissions. Entries with incomplete release forms and/or containing libel or slanderous content will be automatically disqualified from the video contest. Recognizable public figures do not need to sign the release.
University of Puget Sound may use my name, voice, image, and/or likeness for publicizing and promoting the university through use of these audio and/or video recordings.
I hereby give University of Puget Sound permission to use audio and/or video (including digital media), or otherwise record my name, voice, and/or likeness as specified below, for promotional purposes. I understand that examples of my work and/or these recordings of me may be distributed by, but not limited to, print, Internet, or digital media, broadcast, cable television transmission, and/or any other conceivable form of distribution or promotional mechanism.
I understand that there will be no financial or other remuneration for use of my audio, video, or other recordings of my name, voice, and/or likeness, either for initial or subsequent transmission or playback, and I hereby release University of Puget Sound from any liability resulting from or connected with the publication of such work. Permission is granted for the duration of the media.
I understand that University of Puget Sound cannot control unauthorized use of my image by persons not associated with the university once it has been published. I hereby waive any right to inspect or approve any publication of my image by University of Puget Sound. I voluntarily and irrevocably give my consent and agree to this release and waiver in perpetuity.
