Acknowledgment of the Rules and Terms of the
MTSU Student Union Game Room
Welcome to the MTSU Student Union Game Room. In order to better serve our patrons we are
requesting that all Game Room users complete the following acknowledgement of the rules
and terms. A sticker will then be placed on your ID to indicate the semester in which you have
filled out this agreement.
As a MTSU student, I understand that I am responsible for reading and following all rules
outlined in Policy III:00:03 Student Disciplinary Rules (http://www.mtsu.edu/policies/studentaffairs/III-00-03.php). Failure to comply with the Student Disciplinary Rules may result in a
referral to Judicial Affairs and Mediation Services for further disciplinary action.

I understand that in order to use the Game Room I must have a valid MTSU ID Card OR be a
guest of a (valid) MTSU ID Cardholder (limit 1 guest at a time). Anyone without a valid MTSU ID
OR is not a guest of a (valid) MTSU ID Cardholder must pay the fee of $5.00/day or
$20.00/week. ____________ (initial)
I understand that if I allow someone who is not myself to use my MTSU ID Card I will be
referred to Judicial Affairs and Mediation Services for disciplinary action. ____________ (initial)
I understand that if I use a MTSU ID Card that is not my own I will be referred to Judicial Affairs

In addition to the Student Disciplinary Rules, I understand that I am responsible for abiding by
the rules and terms of the MTSU Student Union Game Room.
Please read and initial the following Game Room Rules of Conduct:
1. Be respectful of all Game Room staff and patrons. No cursing, name calling, or other
inappropriate language/behavior is permitted. ____________ (initial)
2. Do not sit, lean, or stand on pool tables for shots. Do not move any gaming tables.

3. No masse or jump shots. Do not aim cue downwards at felt. ____________ (initial)
4. Do not hit ping pong paddles against table. ____________ (initial)
5. Equipment should never leave the Game Room. Do not leave any equipment
unattended. YOU ARE RESPONSIBLE FOR ANY DAMAGES. ____________ (initial)
6. No loud music. Headphones must be used when listening to personal music.

7. No food or drink on pool or ping pong tables. ____________ (initial)
8. Always wear shoes and appropriate attire while in the Game Room and in the MTSU

9. Equipment must be checked out and in by the same person. You may not trade
equipment. YOU ARE RESPONSIBLE FOR ANY DAMAGES. ____________ (initial)
Failure to comply with these rules and terms may result in your immediate removal from the
Game Room and a possible referral to Judicial Affairs and Mediation Services and/or MTSU
Police.
This initialed and signed agreement will remain in the Game Room. At any time you may
request to see your signed copy by asking the Game Room attendant. You will need to
complete an updated, signed copy every semester before you may access the Game Room.
