It is expected that PLUS students are involved on campus and serve in leadership positions in campus
organizations. PLUS students are required to attend FIVE campus events each semester. These may
include, but are not limited to: Music/Theatre/Art Performances, Organization Meetings, Festival Events,
Athletic Competitions, Lectures, CU Succeed, and PAC Events. To receive credit for your attendance, you
must have this form signed by a person (typically faculty or staff) in charge of the event you are attending.
