I understand the TEACH Grant program has specific service obligations I must
meet in order for the award to remain as a grant. Failure to satisfy my
obligation will result in the permanent conversion of the grant to a Direct
Unsubsidized Stafford Loan with interest due from the date of the grant
disbursement. I understand that once a TEACH Grant is converted to a loan, it
cannot be converted back to a grant. I have read all requirements for this
award and meet them to the best of my knowledge.
