WEAVEonline Assessment Worksheet Instructions
Below is a template that is designed to facilitate development of your unit/program plan. It is a
useful tool for gathering basic assessment information in a standard format and then inputting
that information into Weave.
To use the document:

Enter information in the table cells; cells will expand as needed
Hover over blue text for definitions
Analysis Questions and Annual Report sections can be created by your institution, or
omitted
Documents can be saved or printed.
Upload this document into Weave Document Management when you have completed
populating the Weave planning cycles.
