12 months’ employment. The content of the form should be discussed with the Teaching
Fellow before the form is signed.
Where performance is unsatisfactory please indicate areas for improvement and
training needs. The Teaching Fellow must sign the form to confirm that they have been
informed of the required improvements and receive a copy of the form. The form should
then be forwarded to the HR Division.
