The ELMSP is a grant funded project to provide professional development for inservice and preservice content area teachers so that these teachers will be better prepared to improve academic outcomes for English learners (ELs). The ELMSP will provide funding for tuition, fees and books for participants. In return participants are required to maintain collaboration and participation with the ELMSP research team through the term of the grant, to meet all class requirements and maintain good standing academically (3.0 G.P.A.) The conditions for the provision of tuition, fees, and books (where noted) are stated below. Please read through the statements, check each, and sign your name on page two, indicating that you have read, understand, and agree to these terms and conditions. You must satisfy all requirements stated within this agreement in order to remain in the ELMS project and to keep the tuition, fees, and books paid for by the project. If these requirements are not met, your participation in the project will cease and you must refund to the ELMSP any tuition, fees, and books received. Timeframe of your participation and description of your stipend (check all to qualify) Your participation in the project as a student is for the period from the date of your signature to this agreement until the completion of the grant, August 31, 2016. For preservice and inservice participants in the ESL add-on licensure program (Tier 1), the ELMSP will pay for up to six (6) ESL concentration courses, which must be completed by the end of the grant, August 31, 2016. For inservice participants in the Tier 2 option (3-credit hour course), ELMSP will pay for one course, including books, and inservice teachers may qualify for a stipend based on availability of funds. For inservice teachers in the Tier 3 option (10-hour Online Module), the ELMSP will pay all fees associated with this online course. Participating teachers may qualify for a stipend based on availability of funds. All monetary support from ELMSP is contingent upon the availability and continuation of funds. Your initial participation does not include any assurance, obligation, or guarantee of subsequent participation. No promises of future participation have been made. Your participation in the project is only funded by the tuition, and fees you receive for any course(s) you enroll and complete that are part of the ESL academic concentration. There are no benefits associated with your participation in this project beyond receipt of tuition and fees. Unless specifically stated above, the participation period begins after you have signed this agreement and on the first day of your attendance at an ELMSP eligible class. Conditions required for participation in the project and your receipt of tuition, fees, and books: (check all to qualify) If you are a preservice teacher, you must remain enrolled as a full time student during the fall and spring semester of the academic year, perform all your classroom and research responsibilities related to the ELMSP (remain in contact with the grant researchers, including, but not limited to providing an updated working email address) and remain in good standing academically (3.0 GPA minimum). If you are an inservice teacher, you must complete your ELMSP course(s), maintain good standing academically throughout the course(s) (3.0 GPA minimum) and maintain collaboration with the research team through the term of the grant until August 31, 2016. Inservice teachers will make themselves available to participate in research related to the ELMS project, including, but not limited to: allowing the project to conduct classroom observations, facilitating the gathering of student data and responding to surveys. However, the ELMS project will take into consideration teachers’ individual circumstances when requesting research participation. You agree to remain enrolled in a course or courses of study and to maintain participation as outlined in the course(s’) syllabus. Rules governing performance issues or early termination of your participation in the program: (check all to qualify) If you are unable to complete your coursework, research responsibilities or maintain the academic GPA requirements, you will be placed on a one semester probation period. If you fail to improve your performance as required by the probation, you will be terminated from participation in the program. In the event of extraordinary circumstances or difficulties, you may be allowed to take an incomplete in a course. Failure to complete the course after the incomplete grade will result in termination from participation in the program. If you are terminated from the project, you will be responsible for repayment of any tuition, fees, stipend and books you have received up to that date to the University of North Carolina Wilmington.
