It provides a forum to network and gain expertise to facilitate knowledge transfer to other pharmacists and healthcare practitioners to optimize the care of patients with thromboembolic disease. Primary Health Care providers such as community pharmacists and Nurse Practitioners are some of the most accessible healthcare professionals. With evolving roles and scopes of practice, taking a proactive role in managing medications has become more important. Anticoagulation management is a good example of this evolution with more providers involved in both monitoring and managing anticoagulation care. Much has changed over the past decade. Warfarin and low molecular weight heparins still have a place in care for many patients, and several novel oral anticoagulants (NOACs) are now available for use in the prevention of stroke for atrial fibrillation (AF) and the acute treatment/prevention of recurrent venous thromboembolism (VTE). With the plethora of data, community pharmacists and nurse practitioners are well situated to ensure appropriate selection and ongoing monitoring of these agents. This learning event will utilize case vignettes to provide up to date information on anticoagulation therapies covering common thrombosis topics relevant to a community-based primary care practice.
