AFFIDAVIT of STATUS and
REJECTION of SPONSORSHIP
I am a national of a country other than the United States of America. I will be attending the
University of North Carolina Wilmington and one of both of the following is true:
1) My legal admission into and continued stay in the U.S. will be by a U.S. visa other than F or
M. One of the options for my legal admission into and continued stay in the U.S. may be by a
J-1 U.S. visa that is sponsored by an entity other than the University of North Carolina
Wilmington. The parameters, restrictions and requirements of my visa allow me to attend the
University of North Carolina Wilmington, a U.S. institution of higher education.
2) I will be taking courses on-line and will not be seeking legal admission into and continued
stay in the U.S. as part of my matriculation at the University of North Carolina Wilmington.
As such, I do not request nor need the University of North Carolina Wilmington's sponsorship of
the F-1, J-1 or any other type of visa on my behalf.
