2015-2016 Proof of Illinois Residency
All recipients of the State of Illinois Monetary Award Program (MAP) Grant must be able to provide verification of
Illinois residency.

A dependent student is a resident of Illinois if the parent of the dependent-applicant, who is required by the
instructions to complete the Free Application for Federal Student Aid (FAFSA), physically resides within the State
of Illinois and Illinois is his or her true, fixed and permanent home. Dependent students are required to submit
documentation in their Parent’s name with a printed Illinois address as proof of Illinois Residency.

An independent student (Not required to provide parent information on the FAFSA) is a resident of Illinois if
the applicant physically resides within the State of Illinois (at the time of application) and has so resided for a
period of twelve (12) continuous, full months immediately prior to the start of the academic year for which
assistance is requested and Illinois must be his or her true, fixed and permanent home. The student must submit
documents in the student’s name with the dates recorded on the documents indicating the applicant has
physically resided in Illinois for the relevant 12 continuous FULL months prior to the start of the school’s
academic year for which the student is applying. For the 2015 - 2016 academic years, the student must prove
Illinois residency for all 12 months from August 24, 2014 to August 24, 2015.
Preferred Documentation for Proof of Illinois Residency:
 Illinois Driver's License/State ID Issued by the Secretary of State (if valid and issued on/before August 24, 2014
{Date applies only for Independent Students} – may submit copy of expired license with new license to show
continuous residency)
Other Acceptable Documentation for Proof of Illinois Residency:
DEPENDENT STUDENTS: If your parent is unable to provide their Illinois Driver’s License or State ID, they may provide
one of the acceptable documents listed below showing their name and address. (NOTE: Dates do not apply to
Dependent Students.)
INDEPENDENT STUDENTS: If your Illinois Driver’s License/State ID was issued after August 24, 2014, you may combine
your Illinois Driver’s License/State ID with any combination of the documents listed below to prove your continuous
Illinois residency for the twelve (12) month period from August 24, 2014 through August 24, 2015. If a document does
not cover the full twelve month period, it will be used as verification for the month dated on the document.
Illinois Auto Registration Card (dated between August 24, 2014 and August 24, 2015); or
Illinois Voter's Registration Card (dated between August 24, 2014 and August 24, 2015); or
Signed 2014 Illinois 1040 Individual Income Tax Return (will only verify 2014 residency); or
Utility or rent bills (copy of utility (water, electric, gas, cable) bills) - One or multiple bills combined must cover
all 12 months from August 24, 2014 – August 24, 2015); or
 Rent or lease agreement (one or multiple combined covering the months of August 24, 2014 - August 24, 2015);
or
 Statement of Benefits from the Illinois Department of Employment Security or Public Aid (covering the months
of August 24, 2014 - August 24, 2015); or
 Statement of Benefits from the Social Security Administration (covering the months of August 24, 2014 - August
24, 2015)




If an applicant is a resident of Illinois, but is unable to provide the appropriate documentation, the applicant may verify residency
through ISAC's appeal process.
