The Nursing Department Mentor/Mentee
MENTORING: Mentorship has been defined as an influential factor in developing and
retaining new faculty.
The Nursing Department has a mentoring program for new faculty whereby each new faculty
member is assigned a mentor for the first three semester of their hire.
The Mentor:
An experienced faculty, knowledgeable in the art and science of teaching and learning as well as
in the culture of College of DuPage, will guide the mentee in development in the role of faculty.
Efforts include providing guidance and resources, ideally through mutually agreed upon times
and venues (i.e. email, face-to-face, phone calls, etc.)
The Mentee:
A new faculty to the College of DuPage Nursing Department, establishes goals with the mentor
to obtain knowledge, skills and abilities in the role of faculty. The mentee engages in the
relationship by preparing for meetings with the mentor, taking advantage of appropriate
resources, and using feedback appropriately. Note: This procedure may be applicable to faculty
who experience a significant change in assignment, also.

Use this handbook to guide the orientation and mentoring of new faculty. Numerous resources
exist, including such things as communication sources and clinical instruction checklist.

Basic Nurse Asst. Training (BNAT) Program Info.
*Forms will be distributed in the organized folder from the department before the class begins,
electronically or will be retrieved from the students' BNAT Handbooks.
Content calendar: Follow template on Nursing Portal for preparing a content calendar.
Submit to the department office for placement on web site.
Reminders:
CPR and Fingerprinting dates will be provided to you via email by the Division Office for
inclusion on the Content Calendar.
Skill Exams I & II are to be scheduled on a "lab" day.
Grade/Attendance: Prepare a hardcopy and electronic attendance and grade sheet per college
policy. The electronic version of the grades and attendance record must be submitted to
the department at the end of the course.
Criminal Background Check Form: Form completed on first day of class and turned in to
department office. The Fingerprinting CBC will be a scheduled day/time for each class.
Students must bring the fee for Accurate Biometrics.
Competency Exam Registration Form: Form can be completed early in course. Codes for the
program and test site may be found on the Nursing Portal. Turn in to the department
office by a designated day at the end of class.
Certificate Petition: Student must complete this online. Directions can be given to the students
at the beginning of the course or at the end.
Blood Borne Pathogens & HIPAA "in-service": Show the films during the first week of class
and have students sign sheet. Turn in the form to the department office.
BNAT Handbook: Includes the program syllabus, IDPH skills guidelines, and Alzheimer's
Disease unit objectives, outline and material. Each student must have their own
handbook. These can be purchased in the bookstore, or printed from the nursing dept.
web site or Blackboard. Review the handbook on the first day.

Classroom Evaluation: These forms are provided in the course folder They are to be turned in
at the end of course to the department office (Folder).
Clinical EvaluationThese forms are provided in the course folder. They are to be turned in at
the end of course to the department office (Folder).
SBAR form: A copy of this form is in the BNAT Handbook and can be retrieved electronically
from the Nursing Portal. Refer to the Nursing Department Orientation and Information
Handbook for proper use.
Course Requirements, Program Policies and Discipline Procedure Acknowledgement:
This form is found in the BNAT handbook. It must be signed by the student and
returned the first day of class. Turn in the form to the department office.
Midterm Verification and Final Grade: The "lead" instructor for each class will complete the
midterm verification and final grade. Refer to the Nursing Department Orientation and
Information Handbook.
Skill Exam 1 and 2: This exam will be scheduled on a lab day by the instructor. The date must
be on the content calendar. An evaluator will be assigned to perform skills competency
testing on occupied bed making, side-lying position, and PROM. The instructor may ask
them to assist with other testing if time is available.
Clinical information: The clinical instructor should share clinical site specific information with
the students. Maps, rules, policies re: parking, meals, storage of belongings, supplies to
bring to site should be included with this information.
Official IDPH Roster: Complete the roster for students that qualify to take the state exam and
submit at the end of the course. Have the form prepared for the skill exam evaluator to
sign on Skill Exam day. This form can be found at www.nurseaidetesting.com.
MyAccess Final Grade Screen shot: After posting final grades in MyAccess, print the actual
MyAccess page or screen shot and submit with the Official IDPH Roster.
Westmont: The move of some BNAT sections to the Westmont Center is an emerging matter
that has the potential to generate changes in policy. See portal: WESTMONT UPDATES.

FAQs
Can I use my personal email?
No, the PT Faculty Handbook states your cod.edu email is the designated
means of department/student communication. You should check this
email at least 3 times per week
Where do I get my lab supplies?
The nursing department has a policy and procedure for the procurement
of lab supplies and the set up for simulation scenarios. Basically, you will
need to notify the lab assistants one week in advance of the scheduled lab
by creating a meeting with the lab assistants in your Outlook Calendar.
Please consult the Policy & Procedure for the steps involved.
What are the lab assistant roles?
The lab assistant roles are to assist FT and PT faculty with the setup of
supplies needed to teach a class. See above instructions for our policy for
requesting supplies. The lab assistants also work with students in the
open lab to reinforce skills learned in lab class and skills used in clinical. If
you have a student in need of skill remediation, please contact Donna and
Priscilla, nursing lab assistants, at 630-942-8398 or email Donna at
perchat@cod.edu and Priscilla at laubscherp@cod.edu and request
remediation time.
How is advising done with students?
Upon admission or readmission to the nursing program, students are
assigned a fulltime faculty advisor. The students have the same advisor
for the duration of the nursing program. The advisor is notified when
students are doing poorly at midterm or are in grade jeopardy. The
advisors make contact with the students by the third week of the
semester and it is up to students to follow up with appointments. If you
are having issues with a specific student in a course, the course leader
should be contacted for specific instructions about what the course
expectations are. If the student has issues outside of the course
i.e. Issues with long term illness or a change in health status, the full time
faculty advisor can be contacted for assistance. The list of full time
faculty advisors can be found on the nursing department portal.
How do I document occurrences with students?
This is done using a tool on the nursing departmental portal labeled the
Student Occurrence Form. The Tool should then be sent to
1. The clinical instructor,
2. the students faculty advisor,
3. a copy is given to the student, and
4. a copy is sent to Peggy Lindstrom, program specialist,
lindstr@cod.edu
The email to Peggy Lindstrom should include the following:
1. The subject line: Student Occurrence form attached
2. The filename to follow this structure: ADN_Student’s last name,
Student’s first name mm/dd/yyyy

How do I get my keys?
Your keys will be ordered for you by the Division office staff. You will pick
your keys up from Public Safety in the Homeland Security Center
building.
What/where is the TLC and how do I find a list of
classes?
The Teaching and Learning Center provides classes, workshops, and
resources for College of DuPage employees. You can find a list of classes
at http://www.cod.edu/tlc/ and click on the Classes/Workshops link. The
TLC is located in SRC 1105/1107.
How do I get help with Blackboard?
The most readily available assistance with Blackboard is through the
Teaching and Learning Center. Their contact information and
programming can be found at http://www.cod.edu/tlc/
How do I access NurseTim webinars?
See page 22.
Who do I submit grades to?
If you are the faculty of record you will submit midterm verification and
final grades on MyAccess. If you are not the faculty of record, you can
submit grades to your lead faculty member.
How do I get a lab coat?
Contact Donna Perchatsch, Lab Assistant, at 630-942-8398 or email her at
perchat@cod.edu. You will have 2 options from our vendor, Scrubs, etc.,
located in Lombard -a short, tailored lab jacket or a longer, physicianstyle jacket in sizes XS, S, M, L, XL, XXL, and XXXL. If you want to try the
jackets on for size, you will need to visit the store in person.

then place the order for you. She will contact you when your lab jacket is
available.
How do I get copies made?
Copies can be made in the part time faculty office in quantities of up to 40
copies. Access code to the printer is 1230. Copies in excess of 40 will need
to be done by the copy center. You will submit a work order online. Go to
http://www.cod.edu/dept/staffservices/Copy.htm and click on the Print
Shop link. First you will need to set up your profile. You can scroll down
until you see that link. Then go to digital print and click on POC Work
Order. Your user ID and password and the same as your email user ID and
password. Browse your computer for the file you want and then click
upload and then complete the rest of the form. The account number for
the copies is 01_10_00226_5909015

There is a part-time faculty office in HSC 1235. The phone number is 630942-3371. This office is staffed Mondays through Thursdays 9:30-1:30 and
3:00-7:00; Fridays 9:30-1:30 and Saturdays 8:00-noon during the academic

behaviors. These can include, but are not limited to Code of Conduct, Clinical/Classroom/Lab
