Donated Food Product Request

The sponsor has read and agrees to adhere to the following criteria established for donated food
products:
(1) Event must be University related.
(2) Donated product-food and/or alcohol-must be provided by licensed food vendor. When event is
scheduled, menu must be provided to UC Conference & Event Scheduling. Product must be
delivered to consumer without charge.
(3) Sponsor must prominently display a sign at the event stating that product is donated by
_________________(licensed food vendor name).
(4) Upon approval, user will be responsible for all handling, storage, delivery, service and disposal of
any remaining product and for general maintenance and damages of service area.
(5) Non-profit organizations receiving donated product need to complete a “Request for License
Exemption Non-Profit Organization Temporary Food Service Establishment” form with the
Missoula City-County Heath Department before event can be confirmed if event is open to the
public.
(6) Opened bottles of donated alcohol not consumed at an event must be disposed of after the
event. The food vendor and/or sponsor is responsible for the disposal of any unopened bottles
of alcohol after an event.
