Lourdes University is a Catholic, co-educational, fouryear liberal arts institution sponsored by the Sisters of St. Francis, Sylvania, Ohio. Lourdes University accepts applications for the CCP Program for both fall and spring semester. Once you have registered for your CCP classes, please make sure you submit a copy of your schedule to your high school Guidance Office for their records, and to make sure that your schedule can accommodate your CCP classes. 18 ACT or 900 SAT (if there is no test score, Lourdes will look at PLAN, PSAT, COMPASS, or university placement testing) Return all forms to: Lourdes University; Office of Admissions; 6832 Convent Blvd.; Sylvania, OH 43560. The following forms must be completed as part of the application process: 1. Admissions Application 2. Agreement Signature Form 3. Student Information Form 4. Release Form for Academic Records 5. Official High School Transcript (Contact your high school Guidance Office to request this.) 6. Test scores (ACT, SAT, PLAN, PSAT, COMPASS or university placement test) Upon review of the required information, you will receive an Admission Decision Letter. Admission is selective. CCP students wishing to continue at Lourdes University following high school graduation need to notify the Office of Admissions of your intention to continue enrollment. Please complete a new application and submit an official copy of your final high school transcript and ACT/SAT scores. Your final high school transcript will be needed upon graduation. Academic Advising and Registering for Classes All CCP students will work with an CCP Academic Advisor in order to register for classes. After receiving confirmation of acceptance into the CCP program, call the Admissions Office, 419-824-5291, for an appointment. Advising must be scheduled prior to each semester. CCP students are responsible for adhering to the same policies and procedures as all non-CCP students, as set forth by Lourdes University. All CCP students must maintain a 2.0 GPA at Lourdes. Failure to do so will result in a student being ineligible to register for one semester. Students need to be forthright with their Academic Advisor about strengths and weaknesses. We want this to be a positive, enjoyable, and enriching experience for post-secondary students. This, however, is only possible if we all work together [e.g., if a student knows that (s)he reads slowly, but comprehends well, we would not want to place that student in too many reading-intensive courses.] All CCP students must complete a Release Form for Academic Records. This will allow the University to submit grades and relevant information to the high school and school district as required by law. Orientation All CCP students must attend a mandatory Orientation at Lourdes University. Students will be notified of dates and times for this Orientation. Parents are encouraged to attend as well. Finances Tuition for CCP students is covered by State funding. Supplies (including art supplies) are the responsibility of the student. Books are paid by the high school and are on loan to the student for the semester. Students may write in and/or highlight text within the books; however, books are collected at the end of each semester. Parking fees are paid by the student. As mentioned above, the tuition is covered EXCEPT in the following situations: • If a student fails a course for any reason, the school district is within its right to pass the tuition bill on to the family of the student. • If a student chooses to drop a class once the semester has begun, (s)he will be responsible for the tuition for that class.
