Instructions can be found by Keyword “degree evaluation” from the Edinboro Homepage (www.edinboro.edu). 3. Take this application and a copy of your degree evaluation to your advisor for signature. 4. After obtaining your advisor’s signature, take the application and a copy of your degree evaluation to the appropriate Department Chair (see reverse side of form) to develop a plan of study for teacher certification. After you have developed the plan of study, take this application, the degree evaluation and plan of study to the Dean of Education for signature. 5. The Office of the Dean of Education will submit this form to the Records and Registration.
